Indications/Uses
Treatment of biliary tract, bone & joint, GUT, resp tract, skin & soft tissue infections; endocarditis, septicaemia. Prophylaxis for peri-op patients undergoing contaminated or potentially contaminated surgical procedures or in whom post-op infection would be serious.
Dosage/Direction for Use
IM/IV Adult & elderly Mild infection caused by gram +ve cocci 250-500 mg 8 hrly. Moderate to severe infections 500 mg-1 g 6-8 hrly. Severe life-threatening infection eg, endocarditis or septicaemia 1-1.5 g 6 hrly. Acute uncomplicated UTI 1 g 12 hrly. Pneumonia due to pneumococcal infection 500 mg 12 hrly. Prophylactic use in surgery 1 g 30-60 min prior to surgery, followed by 500 mg-1 g 6 hrly post-op for 24 hr. If surgery is >2 hr, administer an additional dose of 500 mg-1 g during the surgical procedure. Patient who have undergone open heart surgery or prosthesis implantation Prolong prophylactic administration for 3-5 days post-surgery. Childn Mild to moderate infections Total daily dose: 25-50 mg/kg body wt given as 3 or 4 equally divided doses. Severe infection Total daily dose: Up to 100 mg/kg body wt given as 3 or 4 equally divided doses.
Contraindications
Hypersensitivity to cephalosporins.
Special Precautions
Previous hypersensitivity reactions to penicillins. May cause false +ve Coomb's test. Monitor for pseudomembranous colitis. Drug w/drawal in severe cases. Discontinue use if an allergic reaction occurs. Patients w/ history of GI disease especially colitis. Monitor for signs of superinfection during long-term therapy. Renal impairment. Intrathecal is not an approved route of administration. Pregnancy & lactation. Infants & neonates <1 mth.
Adverse Reactions
Phlebitis at inj site (IV); pseudomembranous colitis; anorexia, diarrhoea, nausea, oral candidiasis & vomiting; genital & anal pruritus, genital candidiasis & vaginitis; +ve direct & indirect Coomb's test, leucopenia, neutropenia, thrombocythaemia, sporadic cases of hypoprothrombinemia (reversible by vit K), thrombocytopenia; anaphylaxis, drug fever, eosinophilia, rash, Stevens-Johnson syndrome, vulvar pruritus; transient elevation of BUN.
Drug Interactions
Increased risk of renal toxicity w/ ethacrynic acid, frusemide & any potent diuretic. Increased & prolonged blood levels (due to decreased renal tubular secretion) w/ probenecid.
