Prazosin HCl
Indications/Uses
Monotherapy or in combination w/ diuretic &/or other classes of antihypertensives in management of HTN. Symptomatic treatment of urinary obstruction caused by benign prostatic hypertrophy (BPH).
Dosage/Direction for Use
HTN Initially 0.5 mg bid or tid for 3-7 days. May be increased to 1 mg bid or tid for further 3-7 days, & gradually increase according to patient's response to max dose of 20 mg daily in divided doses. Combination w/ diuretics or other antihypertensives: Reduce dose to 1 mg or 2 mg tid & gradually increase according to patient's response & tolerance. Patient receiving other antihypertensives w/ inadequate control Reduce the other drug to maintenance dose level. Initiate 0.5 mg in evening, then continue w/ 0.5 mg bid or tid. BPH Initially 0.5 mg bid, & increase to maintenance max dose of 2 mg bid. Moderate to severe renal & hepatic impairment Initially 0.5 mg daily. May be increased cautiously.
Administration
May be taken with or without food: Starting dose is best taken w/ dinner, at least 2-3 hr before retiring. Maintenance doses may be taken w/ or w/o meals.
Contraindications
History of hypersensitivity to prazosin or any other quinazolines eg, alfuzosin, doxazosin, terazosin.
Special Precautions
Postural hypotension evidenced by dizziness & weakness or rarely loss of consciousness. May augment antihypertensive effect of medications known to lower BP. May affect ability to drive & use machines. Moderate to severe renal impairment. Pregnancy & lactation. Not recommended in childn <12 yr. BPH: Not recommended in patients w/ history of micturition syncope.
Adverse Reactions
Dizziness, drowsiness, orthostatic hypotension, headache, lack of energy, nausea, palpitation.
Drug Interactions
Increased hypotensive effect w/ diuretics or other hypotensives, particularly β-blockers eg, propranolol. Additive hypotensive effect & symptomatic hypotension w/ phosphodiesterase type 5 inhibitor eg, sildenafil, tadalafil, vardenafil. Concomitant use w/ protein-bound drugs.
