Adult (including elderly) 250 mg tds, increasing to 500 mg tds for more severe infections.
Severe or recurrent purulent resp tract infection 3 g bd.
Simple acute UTI Two 3 g doses w/ 10-12 hr between doses.
Dental abscess Two 3 g doses w/ 8 hr between doses.
Gonorrhoea Single 3 g dose.
H. pylori eradication in PUD (duodenal & gastric) Recommended at a dose of bd in association w/ a PPI & antimicrobial agents: Omeprazole 40 mg daily, amoxicillin 1 g bd, clarithromycin 500 mg bd for 7 days; or omeprazole 40 mg daily, amoxicillin 750 mg to 1 g bd, metronidazole 400 mg tds for 7 days.
Childn up to 10 yr 125 mg tds, increasing to 250 mg tds for more severe infections;
3-10 yr Severe or recurrent acute otitis media 750 mg bd for 2 days, may be used as alternative course of treatment.
Renal impairment in adult & childn >40 kg: Moderate (CrCl 10-30 mL/min) Max: 500 mg bd,
severe (CrCl <10 mL/min) Max: 500 mg daily
. Childn <40 kg: Moderate (CrCl 10-30 mL/min) 15 mg/kg bd, max: 500 bd,
severe (CrCl <10 mL/min) 15 mg/kg once daily, max: 500 mg.
Patient receiving peritoneal dialysis Severe renal impairment dosing.
Patient receiving haemodialysis Severe renal impairment dosing. May administer an additional dose (500 mg for adult or 15 mg/kg for childn <40 kg) during & at the end of each dialysis.
Endocarditis prophylaxis Individualized dosage.
