Dexmedetomidine HCl
Indications/Uses
Sedation of initially intubated & mechanically ventilated patients during treatment in intensive care setting; non-intubated patients prior to &/or during surgical & other procedures.
Dosage/Direction for Use
IV infusion Individualized dosage. Adult ICU sedation Initiation: 1 mcg/kg loading infusion over 10 min. Maintenance: 0.2-0.7 mcg/kg/hr. Procedural sedation Initiation: 1 mcg/kg loading infusion over 10 min. 0.5 mcg/kg loading infusion over 10 min for less invasive procedures eg, ophth surgery. Maintenance: 0.6 mcg/kg/hr & titrated to desired clinical effect w/ dose ranging from 0.2-1 mcg/kg/hr. Awake fiberoptic intubation Initiation: 1 mcg/kg loading infusion over 10 min. Maintenance: 0.7 mcg/kg/hr until the endotracheal tube is secured. Patients >65 yr Initiation of procedural sedation 0.5 mcg/kg loading infusion over 10 min.
Special Precautions
Decrease or stop infusion, increase rate of infusion, elevate lower extremities & use pressor agents if hypotension, bradycardia & sinus arrest occur. Not to be infused for >24 hr. Tolerance, tachyphylaxis & dose-related increase in adverse events w/ prolonged exposure beyond 24 hr. W/drawal events eg, nausea, vomiting & agitation after prolonged administration (several days); tachycardia &/or HTN. Arousability. Transient HTN during loading dose. Patients w/ advanced heart block &/or severe ventricular dysfunction; hypovolemia, DM or chronic HTN. Monitor patients continuously while receiving treatment. Concomitant administration w/ other vasodilators or -ve chronotropic agents. Hepatic impairment. Not recommended during pregnancy & in women of childbearing potential not using contraception. Lactation. Elderly.
Adverse Reactions
Bradycardia; hypotension, HTN. Hyper-/hypoglycaemia; agitation; myocardial ischaemia or infarction, tachycardia; resp depression; nausea, vomiting, dry mouth; w/drawal syndrome, hyperthermia.
Drug Interactions
May lead to enhanced sedative, anaesth & cardioresp effects w/ anaesth, sedatives, hypnotics & opioids. Enhanced effects w/ isoflurane, propofol, alfentanil & midazolam. Concomitant use of drugs w/ hypotensive & bradycardic effects eg, β-blockers.
