Dexmedetomidine HCl
Indications/Uses
Sedation of adult ICU patients requiring sedation level not deeper than arousal in response to verbal stimulation (corresponding to Richmond Agitation-Sedation Scale (RASS) 0 to -3); non-intubated patients prior to &/or during surgical & other procedures.
Dosage/Direction for Use
IV infusion Individualized dosage. Max duration: 14 days. Initiation of ICU sedation Adult patient already intubated & sedated Initial infusion rate: 0.7 mcg/kg/hr. May be adjusted stepwise w/in dose range of 0.2-1.4 mcg/kg/hr. Maintenance infusion: 0.2-1.4 mcg/kg/hr. Initiation of procedural sedation Awake fiberoptic intubation adult patient Loading infusion: 1 mcg/kg over 10 min. Patient >65 yr & for less invasive procedures eg, ophth surgery Loading infusion: 0.5 mcg/kg over 10 min. Procedural sedation Maintenance infusion: 0.6 mcg/kg/hr. Titrate w/ dose range of 0.2-1 mcg/kg/hr. Awake fiberoptic intubation adult patient Maintenance infusion: 0.7 mcg/kg/hr.
Contraindications
Hypersensitivity. Advanced heart block (grade 2 or 3) unless paced; uncontrolled hypotension.
Special Precautions
Not to be co-administered through same IV catheter w/ blood or plasma. Sinus arrest, bradycardia, hypotension; transient HTN; arousability. W/drawal syndrome in ICU sedation. Tolerance & tachyphylaxis in use beyond 24 hr. Patients w/ advanced heart block &/or severe ventricular dysfunction; hypovolemia, DM, chronic HTN. Consider supportive therapy if tachycardia &/or HTN occurs after treatment discontinuation. Concomitant use w/ vasodilators or -ve chronotropic agents. Hepatic impairment. Pregnancy & lactation. Ped patients. Elderly >65 yr.
Adverse Reactions
Anemia; arrhythmia, ventricular arrhythmia, bradycardia, AV block, cardiac arrest & disorder, MI, extrasystoles, atrial fibrillation, tachycardia, supraventricular & ventricular tachycardia; photopsia, visual impairment; abdominal pain, diarrhea, nausea, vomiting; chills, hyperpyrexia, pain, pyrexia, thirst; abnormal hepatic function, hyperbilirubinemia; increased γ-glutamyl transferase, ALT & AST, blood alkaline phosphatase, blood urea, ECG T wave inversion, prolonged ECG QT; acidosis, hyperkalemia, hypoglycemia, hypovolemia, hypernatremia; convulsion, dizziness, headache, neuralgia, neuritis, speech disorder; agitation, confusional state, delirium, hallucination, illusion; oliguria, polyuria; apnea, bronchospasm, dyspnea, hypercapnia, hypoventilation, hypoxia, pulmonary congestion, resp acidosis; hyperhidrosis, pruritus, rash, urticaria; light anesth; hemorrhage, BP fluctuation, HTN, hypotension.
Drug Interactions
Enhanced effects w/ anesth, sedatives, hypnotics, opioids; sevoflurane, isoflurane, propofol, alfentanil, midazolam.
