Indications/Uses
Nosocomial & community-acquired pneumonia including concurrent bacteremia; complicated & uncomplicated skin & skin structure infections; vancomycin-resistant Enterococcus faecium infections including concurrent bacteremia.
Dosage/Direction for Use
Adult & adolescent ≥12 yr Nosocomial & community-acquired pneumonia, complicated skin & skin structure infections 600 mg IV or PO every 12 hr for 10-14 days. Vancomycin-resistant Enterococcus faecium infections 600 mg IV or PO every 12 hr for 14-28 days. Uncomplicated skin & skin structure infections Adult 400 mg PO every 12 hr for 10-14 days. Adolescent 600 mg PO every 12 hr for 10-14 days. Childn birth through 11 yr Nosocomial & community-acquired pneumonia, complicated skin & skin structure infections 10 mg/kg IV or PO every 8 hr for 10-14 days. Vancomycin-resistant Enterococcus faecium infections 10 mg/kg IV or PO every 8 hr for 14-28 days. Uncomplicated skin & skin structure infections Childn 5-11 yr 10 mg/kg PO every 12 hr for 10-14 days, <5 yr 10 mg/kg PO every 8 hr for 10-14 days. Premature neonate <7 days 10 mg/kg every 12 hr.
Administration
May be taken with or without food.
Contraindications
Hypersensitivity. Uncontrolled HTN, pheochromocytoma, thyrotoxicosis, carcinoid syndrome. Concomitant use w/ directly & indirectly acting sympathomimetic (eg, pseudoephedrine, phenylpropanolamine), vasopressive (eg, epinephrine, norepinephrine) & dopaminergic agents (eg, dopamine, dobutamine), serotonin reuptake inhibitors, TCAs, serotonin 5-HT1 receptor agonists, meperidine or buspirone. Not to be used concomitantly w/in 2 wk of MAOIs.
Special Precautions
Discontinue use if serotonin syndrome occurs. Not to be used in patients w/ catheter-related bloodstream infection. Pseudomembranous colitis; Clostridium difficile-associated diarrhea; lactic acidosis; convulsions; hyponatremia, SIADH. Monitor CBC in patients at increased bleeding risk, preexisting myelosuppression; visual function in patients on long-term therapy & w/ new visual symptoms. Concomitant use w/ serotonergic agents eg, antidepressants eg, SSRIs, opioids; medications decreasing Hb levels, platelet count or function. Hepatic & severe renal insufficiency. Pregnancy & lactation. Monitor serum Na levels in elderly.
Adverse Reactions
Moniliasis; thrombocytopenia, anemia; headache; vomiting, diarrhea, nausea, abdominal pain & cramps; rash; abnormal LFTs.
Drug Interactions
Mild reversible enhancement on pressor response induced by pseudoephedrine or phenylpropanolamine HCl. Serotonin syndrome w/ serotonergic agents. Decreased Cmax & AUC w/ rifampicin. Infusion: Physically incompatible w/ amphotericin B, chlorpromazine HCl, diazepam, pentamidine isethionate, erythromycin lactobionate, phenytoin Na, trimethoprim-sulfamethoxazole. Chemically incompatible w/ ceftriaxone Na.
