Colchicine
Indications/Uses
Acute gout. Prophylaxis of recurrent gout. Prevention of acute attacks during initial therapy w/ allopurinol & uricosuric drugs.
Dosage/Direction for Use
0.5 mg Acute gout Initially 1 mg then 0.5 mg every 2-3 hr until pain relief or diarrhoea/vomiting occurs or until 6 mg total dose is reached. Not to repeat course of treatment w/in 3 days. Prophylaxis 0.5 mg bd or tds. 0.6 mg Acute gout & pseudogout 1-2 tab every 2 hr until pain relief or nausea/vomiting/diarrhea occurs. Acute gout prophylaxis 1 tab daily 1-4 times wkly to 1 tab daily. Pseudogout prophylaxis Up to 2 tab daily.
Administration
May be taken with or without food.
Contraindications
Hypersensitivity. Blood dyscrasias, uric acid kidney stones. Patients undergoing haemodialysis. Pregnancy & lactation. Childn <2 yr.
Special Precautions
Cardiac or GI disease. Renal & hepatic disease. Elderly & debilitated patients.
Adverse Reactions
Nausea, vomiting, diarrhea & abdominal pain.
Drug Interactions
P-gp or strong CYP3A4 inhibitors eg, clarithromycin, cyclosporin, erythromycin, verapamil & diltiazem, telithromycin, ketoconazole, itraconazole, HIV PIs & nefazodone. Increased serum uric acid w/ thiazide diuretics. Impaired vit B12 absorption. Colchicine toxicity w/ tolbutamide. Avoid use w/ grapefruit & grapefruit juice.
