Furosemide
Indications/Uses
Edema associated w/ CHF, hepatic cirrhosis & renal disease including nephrotic syndrome. Tab: Mild to moderate HTN. Inj: Adjunct in acute pulmonary edema & hypertensive crisis.
Dosage/Direction for Use
Tab Edema Initially 40 mg once daily. Mild cases 20 mg daily or 40 mg on alternate days. Severe cases Gradual titration up to 600 mg daily. Childn 1-3 mg/kg daily. Max: 40 mg daily. HTN 40-80 mg daily. Chronic renal insufficiency Initially 250 mg daily, may be increased by 250 mg every 4-6 hr up to max: 1.5 g in 24 hr; in exceptional cases, 2 g in 24 hr. Inj Pulmonary oedema Initially 40 mg slow IV inj over 1-2 min, may be increased to 80 mg. Childn 0.5-1.5 mg/kg daily. Max: 20 mg daily. HTN 40-80 mg daily. Oliguria in acute or chronic renal failure 250 mg diluted to 250 mL infused over 1 hr.
Administration
Should be taken on an empty stomach: If GI distress occurs, take w/ food or milk.
Contraindications
Hypersensitivity to sulfonamides. Anuria. Renal failure w/ azotemia &/or oliguria. Hepatic coma or electrolyte depletion.
Special Precautions
Not to be given in anuria or renal failure due to nephrotoxic or hepatotoxic drugs nor in renal failure associated w/ hepatic coma; pre-comatose state associated w/ hepatic cirrhosis. Prostatic hypertrophy or micturition impairment. Monitor fluid & electrolyte regularly. Pregnancy. Not to be given during lactation.
Adverse Reactions
Fluid & electrolyte imbalance including hyponatraemia, hypokalaemia & hypochloraemic alkalosis; nephrocalcinosis in preterm infants; pupura, photosensitivity, rash, urticaria, pruritus, exfoliative dermatitis, erythema multiforme & necrotizing angiitis eg, vasculitis & cutaneous vasculitis.
Drug Interactions
Enhanced nephrotoxicity of cephalosporin antibiotics eg, cephalothin. Enhanced ototoxicity of aminoglycoside antibiotics. Enhanced hypotensive action of antihypertensive drugs. Marked BP reduction w/ ACE inhibitors. Risk of hearing defects w/ cisplatin. K-lowering effect w/ steroids. Provoked lithium toxicity. Reduced absorption w/ phenytoin. Antagonized natriuresis & increased plasma renin activity w/ NSAIDS. Increased serum conc by probenecid.
