N-acetylcysteine
Indications/Uses
All resp tract diseases leading to formation of thick secretions difficult to be expectorated eg, acute & chronic bronchitis, laryngitis, sinusitis, tracheitis, flu, bronchial asthma & (as complementary treatment) mucoviscidosis.
Dosage/Direction for Use
Adult & adolescent ≥12 yr 600 mg daily, divided into 1 or more administrations (eg, 200-mg sachet tds, 10 mL tds or 30 mL daily). Childn 2-12 yr One 100-mg sachet tds or 200 mg bd or 5 mL tds or 10 mL bd. Long-term treatment 400-600 mg daily, divided into 1 or more administrations. Max duration: 3-6 mth. Mucoviscidosis Same as previous dose except: Childn from 6 yr 200 mg tds or 600 mg once daily.
Administration
May be taken with or without food: Effervescent tab/powd for oral soln: Dissolve content of 1 sachet/effervescent tab into glass of cold/warm water.
Contraindications
Special Precautions
Discontinue use if hypersensitivity reactions or bronchospasm occur. Histamine intolerance. Not to suppress productive cough (fundamental part of bronchopulmonary defence mechanism). Not rational to combine mucolytic w/ cough suppressant medicine &/or substance drying out secretions (atropinic drug). Peptic ulcer or history of PUD especially w/ concomitant use of medicinal products irritating GI tract mucous membranes. Patients w/ bronchial asthma & hyperreactive bronchial system due to risk of bronchospasm. Bronchial secretions may become more fluid & increase in vol particularly at start of treatment. Perform postural drainage & bronchoaspiration if patient is unable to cough up secretions effect. Not recommended to be used w/ antitussive drugs. Pregnancy. Not to be used during lactation. Not to use mucolytic agents in infants since mucolytic agents can cause excessive bronchial congestion in infants due to limited capacities of bronchial mucus drainage by specific physiological airways characteristics. 100, 200 & 600 mg: Phenylketonuria. Not to be taken by patients w/ galactose & fructose intolerance, total lactase deficiency or glucose-galactose malabsorption. 600 mg: Patients on low Na diet. Not to be used in childn <6 yr w/ cystic fibrosis & childn <12 yr. Syr: May cause allergic reactions due to methyl parahydroxybenzoate.
Drug Interactions
Decreased effect w/ activated charcoal. Severe hypotension & increased temporal artery vasodilation w/ nitrate derivatives. Allow 2 hr interval prior to oral antibiotics or other medicinal products (except loracarbef). Concomitant use w/ carbamazepine. Syr: May interfere w/ colorimetric analysis method & ketonuria assay.
