Contents
Metronidazole 500 mg, nystatin 100,000 IU.
Action
Metronidazole is bactericidal, amoebicidal and trichomonaside. The mechanism of action cannot be assured yet.
Metronidazole is not ionized at physiological pH and is ready to be taken by organisms or anaerobic cells. In susceptible organisms or cells, Metronidazole undergoes a reduction process by low redox potential electron transport proteins (eg, nitroreductase such as ferredoxin) into polar products that have lost the nitro group so that it is not known. This reduction product is responsible for the cytotoxic and antimicrobial effects of Metronidazole, which includes interference with DNA and inhibition of nucleic acid synthesis. Metronidazole is as effective as cells whether it is splitting or not.
Nystatin acts as a fungistatic and fungicides, it works by binding the sterols (especially ergosterol) in the cell membranes of fungi. Nystatin is not active against organisms (eg, bacteria) that do not have sterols on their cell membranes. The results of this bond make the cell membrane no longer function as a selective barrier, and potassium and other components will disappear. The main effectiveness of Nystatin is to against Candida (Monilia) spp.
Indications/Uses
Vaginitis caused by Trichomonas vaginalis and Candida albicans.
Dosage/Direction for Use
One ovule daily for 7-10 days, insert into the vagina before bed-time. The ovule must be inserted deep enough into the vagina, preferably using the applicator.
Contraindications
Hypersensitivity to Metronidazole and Nystatin and other components. Pregnancy in the first trimester.
Special Precautions
In patients with blood dyscrasias, total and differential leukocyte counts must be monitored before and after therapy.
If there is irritation, use of drugs is decreased or stopped using it altogether. It is recommended not to have vaginal intercourse, and stop vaginal douche during therapy.
Caution in administration to patients with hepatic encephalopathy (it may cause high plasma concentrations of Metronidazole and cause symptoms of encephalopathy).
Peripheral neuropathy and epilepsy are serious side effects that may occur in the nervous system due to the high-dose or because of long-term usage of Metronidazole.
Elders. Pregnancy & Lactation.
Adverse Reactions
Hypersensitivity. Dryness and burn sensation in vagina. Itches, urticaria, pruritus, erythema multiforme, angioedema and anaphylactic reaction; nausea, metal taste accompanied by vomiting and anorexia. Diarrhea, dry mouth, furred tongue, glossitis, stomatitis. Colitis pseudomembrane; peripheral neuropathy; leukopenia and thrombocytopenia.
Drug Interactions
Drugs that can interact with Metronidazole are: Coumarin Anticoagulants: The use of Metronidazole extends the effect of oral anticoagulants causing prolonged prothrombin time or blood clotting, so avoid using Metronidazole with anticoagulants.
Metronidazole can interact with alcohol causing reactions such as disulfiram (a disulfiram - like reaction) in some patients. Be careful about using alcohol while on therapy with Metronidazole.
Co-administration with disulfiram can cause acute psychosis and confusion (confusion) in some patients. Therefore, the two drugs should not be used together until 2 weeks after disulfiram usage ends.
Short-term therapy with Metronidazole in patients stabilized with relatively high lithium doses is reported to increase the concentration of lithium in serum causing signs of toxicity in some patients.

Store in the temperature below 25°C, in a dry place and protected from the sunlight.
