Indications/Uses
Treatment of osteolytic, osteoblastic & mixed bone metastases of solid tumors & osteolytic lesions of multiple myeloma in conjunction w/ standard antineoplastic therapy. Reduction of bone damage in patients w/ advanced malignancies involving bone. Treatment of hypercalcemia of malignancy (HCM).
Dosage/Direction for Use
IV infusion lasting no <15 min Adult & elderly Treatment of bone metastases & osteolytic lesions & reduction of bone damage in patients w/ advanced malignancies involving bone 4 mg IV infusion lasting no <15 min every 3-4 wk. Administer oral Ca supplement of 500 mg & vit D 400 IU daily. Treatment of HCM 4 mg single infusion.
Contraindications
Hypersensitivity to zoledronic acid or other bisphosphonates. Pregnancy & lactation.
Special Precautions
Assess patients w/ mild to moderate renal impairment if adequately hydrated prior to administration. Severe renal or hepatic impairment. Monitor serum Ca, phosphate, Mg & creatinine. Ensure adequate hydration. Avoid invasive dental procedures in patients w/ concomitant risk factors (eg, cancer, chemotherapy, corticosteroid therapy, poor dental status) due to risk of osteonecrosis of the jaw. Monitor for atypical femur fracture, severe & occasional incapacitating bone, joint &/or muscle pain, hypocalcemia.
Adverse Reactions
Anemia; headache, paresthesia; sleep disorder; conjunctivitis; nausea, vomiting, anorexia, constipation; hyperhidrosis; bone pain, myalgia, arthralgia, generalized pain; HTN; renal impairment; acute phase reaction, pyrexia, flu-like illness, peripheral edema, asthenia; hypophosphatemia; increased blood creatinine & urea, hypocalcemia.
Drug Interactions
Additive effect w/ aminoglycosides & loop diuretics. Other potentially nephrotoxic & antiangiogenic drugs.
