Valaciclovir HCl
Indications/Uses
Herpes zoster (shingles). Treatment or suppression of genital herpes in immunocompetent individuals w/ & for suppression of recurrent genital herpes in HIV-infected individuals. Cold sores (Herpes labialis).
Dosage/Direction for Use
Herpes zoster 1 g tid for 7 days. Genital herpes Initial episodes: 1 g bid for 10 days. Recurrent episodes: 500 mg bid for 3 days. Suppressive therapy: Patients w/ healthy immune function 1 g once daily; w/ history of ≤9 recurrences per yr & alternative dosage 500 mg once daily. Chronic suppressive therapy of recurrent genital herpes in HIV-infected patients w/ CD4 cell count at least 100 cells/mm3 500 mg bid. Reduction of transmission: Patients w/ history of ≤9 recurrences per yr 500 mg once daily for source partner. Cold sores (Herpes labialis) 2 g bid, taken about 12 hr apart.
Administration
May be taken with or without food.
Contraindications
Hypersensitivity or intolerance to valaciclovir, aciclovir.
Special Precautions
TTP/haemolytic uremic syndrome (HUS) in patients w/ advanced HIV infection & in allogenic bone marrow or renal transplant recipients receiving high dosage (8 g daily). Pregnancy & lactation.
Adverse Reactions
HTN, tachycardia; depression, dizziness, headache, aggressive behaviour, agitation, ataxia, coma, confusion, decreased consciousness, dysarthria, encephalopathy, mania, psychosis including, auditory & visual hallucinations, seizures, tremors; rash, alopecia, erythema multiforme, rashes including, photosensitivity; abdominal pain, nausea, vomiting, diarrhea; aplastic anemia, leukocytoclastic vasculitis, thrombocytopenia, TTP/HUS; hepatic, liver enzyme abnormalities; acute hypersensitivity reactions including anaphylaxis, angioedema, dyspnea, pruritus & urticaria; visual abnormalities; elevated creatinine, renal failure; facial edema.
Drug Interactions
Consider dosage adjustment in concomitant use w/ Al- or Mg-containing antacids & digoxin. Increased peak plasma conc & AUC w/ cimetidine & probenecid.
