Indications/Uses
Acute relief of migraine attacks w/ or w/o aura, including treatment of migraine attacks associated w/ menstrual period in women.
Dosage/Direction for Use
Adult Initially 50 mg as a single dose. May repeat dose after 2 hr, & not to exceed a total daily dose of 200 mg if headache returns or patient has a partial response to initial dose. Max: 200 mg daily. Mild to moderate hepatic impairment Not to exceed single dose of 50 mg.
Administration
May be taken with or without food.
Contraindications
History of hypersensitivity & stroke or transient ischemic attack, peripheral vascular disease, or ischemic bowel disease. Uncontrolled HTN; hemiplegic or basilar migraine; ischemic heart disease (angina pectoris, history of MI, or documented silent ischemia); coronary artery vasospasm (eg, Prinzmetal variant angina). Concomitant use of ergotamine/derivatives of ergotamine (including methysergide). Concurrent administration w/ MAOls.
Special Precautions
Exclude potentially serious neurological conditions (eg, CVA, transient ischaemic attack) if patient presents w/ atypical symptoms. Not indicated for use in management of hemiplegic, basilar or ophthalmoplegic migraine. Patients w/ history of seizure disorders or conditions associated w/ lowered seizure threshold. Perform CV evaluation prior to initiation of therapy in patients w/ multiple CV risk factors (eg, postmenopausal women; men >40 yr; w/ risk factor eg, HTN, hypercholesterolemia, obesity, diabetes, smoking, or family history of CAD) who have not previously received therapy w/ 5-HT1 receptor agonist. May affect ability to drive & operate machinery. Patients w/ diseases altering drug absorption, metabolism, or excretion eg, impaired renal/hepatic function. Pregnancy & lactation. Childn & adolescents <18 yr. Not recommended in elderly >65 yr.
Adverse Reactions
Chest pain, pressure, & tightness, hot & cold flashes; fatigue, feeling of heaviness, chest heaviness, malaise, pain, paresthesia, pressure & tightness sensation, vertigo; sore throat; local pain; jaw pain, pressure, & tightness, neck pain; pharyngeal edema.
Drug Interactions
Decreased clearance & increased t½ & blood conc w/ MAO-A inhibitors. Prolonged vasospastic reactions, & additive vasoconstrictor effects w/ ergot alkaloids (eg, ergotamine, dihydroergotamine, methylsergide. 5-HT1 receptor agonists & SSRIs or SNRIs.
