Triamcinolone
Indications/Uses
Anti-inflammation, allergy, RA, acute rheumatic fever, bronchial asthma, allergic rhinitis, dermatoses, acute bursitis, sprue, uveitis & blood dyscrasia.
Dosage/Direction for Use
Adult & ped weighing >60 lbs Initially 8-32 mg daily. Reduce dose by 2 mg every 2-3 days until lowest dose is reached. Ped weighing <60 lbs 4-12 mg daily.
Administration
Should be taken with food.
Contraindications
History of hypersensitivity. Systemic fungal infection. Concomitant use w/ live or attenuated virus vaccines.
Special Precautions
Hypersensitivity. Not to be used in patients w/ systemic fungal infection, unless necessary. Patients w/ peptic ulcer or duodenal ulcer, diabetes, TB or viral infections; cirrhosis; diverticulitis; hypothyroidism; HTN; myasthenia gravis; ocular herpes simplex; osteoporosis. Psychotic tendencies may be aggravated. May increase incidence of secondary infection, mask acute infection (including fungal infection), prolong or exacerbate viral infections. Edema in the presence of renal disease. Withdraw therapy gradually. Renal insufficiency. Pregnancy & lactation.
Adverse Reactions
HTN; atrophic skin condition, impaired wound healing; Cushing's syndrome, decreased body growth; GIT disorder, loss of appetite; osteoporosis; depression, euphoria. Hyperglycemia, primary adrenocortical insufficiency; cataract, glaucoma; pulmonary TB.
Drug Interactions
Enhanced adverse/toxic effect of acetylcholinesterase inhibitors; deferasirox; leflunomide; loxapine; natalizumab; nicorandil; NSAIDs; quinolone antibiotics; live vaccines. Diminished neoplastic effect of aldesleukin. Increased metabolism w/ aminoglutethimide. Enhanced hypokalemic effect of amphotericin B; thiazide diuretics. Enhanced fluid-retaining effect of androgens. Diminished therapeutic effect of antidiabetic agents; BCG; calcitriol; corticorelin; hyaluronidase; sipuleucel-T; urea cycle disorder agents; inactivated vaccines. Increased serum conc w/ aprepitant; strong CYP3A4 inhibitors; estrogen derivatives; fosaprepitant; mifepristone; telaprevir. Enhanced hyperglycemic effect of ceritinib. Diminished diagnostic effect of Coccidioides immitis skin test. Decreased serum conc w/ strong CYP3A4 inducers; mitotane. Enhanced adverse/toxic effect w/ denosumab; pimecrolimus; ritonavir; salicylates; topical tacrolimus. Diminished therapeutic effect w/ echinacea; mifepristone. Enhanced hypokalemic effect w/ indacaterol. Decreased serum conc of INH; telaprevir. Enhanced adverse neuromuscular effect w/ non-depolarizing neuromuscular blocking agents. Enhanced immunosuppressive effect w/ roflumilast. Enhanced immunosuppressive effect of tofacitinib. Enhanced neutropenic effect w/ trastuzumab. Enhanced anticoagulant effect of warfarin.
