Indications/Uses
Duodenal & benign gastric ulceration, GERD, other conditions where gastric acid secretion reduction is likely to be beneficial & pathologic GI hypersecretory conditions eg, Zollinger-Ellison syndrome.
Dosage/Direction for Use
FC tab Duodenal or benign gastric ulcer 800 mg daily as single dose at bedtime or 400 mg bid w/ breakfast & at bedtime for 4-6 wk. Maintenance dose: 400 mg at bedtime, or 400 mg bid. GERD 400 mg qid for 4-12 wk. Symptomatic relief of excessive gastric acid (acid indigestion, heartburn, or sour stomach) 200 mg daily. Max: 400 mg daily. Pathologic GI hypersecretory conditions (eg, Zollinger-Ellison syndrome) 400 mg qid. Ped 20-40 mg/kg daily in divided doses. Inj Adult 300 mg every 6-8 hr via IV or IM inj, or 37.5 mg/hr continuous IV infusion (w/ or w/o 150 mg IV loading dose). Max total daily dose: 2.4 g. Ped 20-40 mg/kg daily in divided doses. Severe renal impairment 300 mg every 12 hr. Alternative doses: GFR 10-50 mL/min Administer 50% of normal dose, <10 mL/min 300 mg every 8-12 hr. Continuous renal replacement therapy Administer 50% of normal dose. Peritoneal dialysis 300 mg every 8-12 hr.
Administration
Should be taken with food.
Contraindications
Special Precautions
Cardiac arrhythmias & hypotension, w/ rapid administration by IV bolus inj. Reversible confusional states, increased risk in severely ill patients. Increased risk of strongyloidiasis hyperinfection in immunocompromised patient. Symptomatic response to therapy does not rule out presence of gastric malignancy. Hepatic impairment. Avoid in pregnancy. Not recommended during lactation. Not recommended in childn <16 yr; <12 yr for non-prescription use. Elderly ≥50 yr. Avoid in older adults w/ or at high risk of delirium.
Adverse Reactions
Diarrhea, GI disturbance, headache, dizziness, tiredness, somnolence, gynecomastia, rash, myalgia, arthralgia. Reversible confusional states; cardiac arrhythmias & hypotension following rapid IV administration; SJS, epidermal necrolysis, erythema multiforme, exfoliative dermatitis, generalized exfoliative erythroderma; agranulocytosis, neutropenia; increased serum AST & ALT; increased plasma creatinine.
Drug Interactions
Increased plasma levels of coumarin anticoagulants (eg, warfarin), phenytoin, propranolol, some benzodiazepines, lidocaine, metronidazole, triamterene, some TCAs & theophylline. Prolonged QT interval & increased risk of life-threatening cardiac arrhythmias, including torsades de pointes cardiac arrest w/ antiarrhythmics eg, disopyramide, dofetilide, procainamide, quinidine & sotalol. Decreased absorption of some azole antifungals eg, ketoconazole. Increased risk of bleeding w/ warfarin. Potentiated myelosuppressive effects of myelosuppressive drugs eg, alkylating agents (eg, carmustine), antimetabolites or therapies (eg, radiation). FC tab: Reduced absorption w/ antacid.
