Indications/Uses
Management of postherpetic neuralgia, pain associated w/ diabetic peripheral neuropathy, & fibromyalgia in adults. In combination w/ other anticonvulsants in management of partial seizure in adults.
Dosage/Direction for Use
Adult Postherpetic neuralgia 150-300 mg daily in 2 or 3 divided doses. Initially 150 mg daily (75 mg bid or 50 mg tid). May be increased to 300 mg daily w/in 1 wk. Pain associated w/ diabetic peripheral neuropathy Initially 150 mg daily in 3 divided doses. May be increased w/in 1 wk to max of 300 mg daily in 3 divided doses. Fibromyalgia 300-450 mg daily. Initially 150 mg daily (75 mg bid). May be increased to 300 mg daily (150 mg bid) w/in 1 wk. Adjunctive therapy in management of partial seizures 150-600 mg daily in 2 or 3 divided doses. Initially 75 mg bid or 50 mg tid (Max: 150 mg daily). May be increased to max of 600 mg daily. Renal impairment w/ CrCl 30-60 mL/min 150 mg normal renal function daily dose: Reduce to 75 mg in 2-3 divided doses, 300 mg: Reduce to 150 mg in 2-3 divided doses, 450 mg: Reduce to 225 mg in 2-3 divided doses, 600 mg: Reduce to 300 mg in 2-3 divided doses; 15-30 mL/min 150 mg normal renal function daily dose: Reduce to 25-50 mg in 1-2 divided doses, 300 mg: Reduce to 75 mg in 1-2 divided doses, 450 mg: Reduce to 100-150 mg in 1-2 divided doses, 600 mg: Reduce to 150 mg in 1-2 divided doses; <15 mL/min 150 mg normal renal function daily dose: Reduce to 25 mg daily, 300 mg: Reduce to 25-50 mg daily, 450 mg: Reduce to 50-75 mg daily, 600 mg: Reduce to 75 mg daily.
Administration
May be taken with or without food.
Contraindications
Special Precautions
Increased risk of suicidal behavior/thoughts. Patients w/ history of angioedema. Monitor for signs/symptoms of depression, suicidal tendencies, & other unusual behavior changes during therapy. Pregnancy & lactation.
Adverse Reactions
Chest pain, edema, hyper-/hypotension; amnesia, anxiety, ataxia, attention disturbance, dizziness; bruising, decubitus ulcer, facial edema, pruritus; fluid retention, hypoglycemia, decreased libido; abdominal distension & pain, increased appetite, constipation, flatulence, nausea, vomiting; impotence, incontinence, urinary frequency; thrombocytopenia; abnormal gait, arthralgia, back pain, balance disorder; blurred vision, conjunctivitis, diplopia; otitis media, tinnitus; bronchitis, nasopharyngitis, pharyngolaryngeal pain, sinusitis; accidental injury, allergic reaction, flu-like syndrome.
Drug Interactions
Avoid concomitant use w/ azelastine, orphenadrine, paraldehyde & thalidomide. Increased levels/effects of alcohol, antidiabetics (thiazolidinedione), azelastine, CNS depressant, orphenadrine, zolpidem. Increased levels/effect w/ doxylamine, hydroxyzine. Decreased levels/effect w/ mefloquine, orlistat.
