Indications/Uses
Prophylaxis of invasive fungal infections, including both yeasts & moulds, in patients ≥13 yr, who are at high risk of developing these infections, eg, patients w/ prolonged neutropenia or hematopoietic stem cell transplant (HSCT). Invasive aspergillosis in patients ≥13 yr w/ disease refractory to amphotericin B, itraconazole or voriconazole, or intolerant to these drugs. Esophageal candidiasis or candidemia in patients ≥13 yr w/ disease refractory to amphotericin B, fluconazole or itraconazole, or in patients who are intolerant of these drugs. Fusariosis, zygomycosis, cryptococcosis, chromoblastomycosis & mycetoma in patients ≥13 yr w/ disease refractory to other therapy, or patients who are intolerant of other therapy. Coccidioidomycosis.
Dosage/Direction for Use
Prophylaxis of invasive fungal infections Loading dose: 300 mg bid on 1st day, then 300 mg once daily thereafter. Patient w/ acute myelogenous leukemia or myelodysplastic syndromes Start several days before anticipated onset of neutropenia & continue for 7 days after neutrophil count rises >500 cells/mm3. Refractory invasive fungal infections/intolerant patient w/ invasive fungal infections, & coccidioidomycosis; oesophageal candidiasis Loading dose: 300 mg bid on the 1st day, then 300 mg once daily thereafter.
Administration
May be taken with or without food: Swallow whole, do not chew/crush/divide.
Contraindications
Hypersensitivity. Co-administration w/ CYP3A4 substrates (terfenadine, astemizole, cisapride, pimozide, quinidine), HMG-CoA reductase inhibitors, ergot alkaloids.
Special Precautions
Hypersensitivity to other azoles. Monitor K, Mg or Ca levels & correct as necessary prior to & during therapy. Concomitant use w/ QTc interval prolonging drugs; vincristine; venetoclax. Hepatic toxicity & impairment. Pregnancy & lactation. Childn <13 yr.
Adverse Reactions
Neutropenia; anorexia, electrolyte imbalance, hypokalemia; dizziness, headache, paresthesia, somnolence; abdominal pain, diarrhoea, dyspepsia, flatulence, dry mouth, nausea, vomiting, constipation; elevated LFTs including AST, ALT, alkaline phosphatase, γ-glutamyl transferase, bilirubin; rash, pruritus; asthenia, fatigue, pyrexia.
Drug Interactions
Decreased Cmax & AUC w/ rifabutin & phenytoin. Increased plasma conc of ergot alkaloids (ergotamine & dihydroergotamine), vinca alkaloids (eg, vincristine & vinblastine), cyclosporine, digoxin. Increase Cmax & AUC of tacrolimus, sirolimus, rifabutin, midazolam, HIV PIs (eg, atazanavir & ritonavir), HMG-CoA reductase inhibitors (eg, simvastatin), venetoclax. Concomitant use w/ Ca channel blockers.
