Memantine HCl
Indications/Uses
Moderate to severe dementia of Alzheimer's type.
Dosage/Direction for Use
Adult Initially 5 mg once daily. Following initiation of therapy, increase to 10 mg daily (5 mg bid), subsequently increase to 15 mg daily (5 mg & 10 mg as separate doses), then 20 mg daily (10 mg bid). Recommended target dose: 20 mg daily in 2 divided doses (10 mg bid). Severe renal impairment (CrCl 5-29 mL/min) 5 mg bid.
Administration
May be taken with or without food.
Contraindications
Special Precautions
Seizures. Conditions that increase urinary pH eg, dietary changes (from a high protein to vegetarian diet); renal tubular acidosis; severe UTI. Concomitant use of urine alkalizers eg, carbonic anhydrase inhibitors, Na bicarbonate. Hepatic impairment. Severe renal impairment. Pregnancy & lactation. Childn. Elderly.
Adverse Reactions
Confusion, dizziness, fatigue, hallucination, headache, somnolence, agitation, anxiety, depression, abnormal gait, insomnia; constipation, vomiting, diarrhea, nausea; coughing, dyspnea, bronchitis, URTI; back pain, HTN, pain, anorexia, arthralgia, fall, inflicted injury, flu-like symptoms, peripheral edema; urinary incontinence, UTI.
Drug Interactions
Minimal inhibition of CYP isoenzymes 1A2, 2A6, 2C9, 2D6, 2E1, or 3A4. Altered plasma conc w/ drugs secreted by the same renal cationic system eg, cimetidine, hydrochlorothiazide, metformin, nicotine, quinidine, ranitidine, triamterene. Decreased clearance w/ agents that increase urine pH eg, carbonic anhydrase inhibitors, Na bicarbonate.
