Indications/Uses
Increases color intensity of skin or helps in eliminating the outer layer of epidermis w/ UV radiation (solar or artificial), or in combination w/ photosensitizing agent for psoriasis & idiopathic vitiligo.
Dosage/Direction for Use
Idiopathic vitiligo Adult & childn >12 yr 20 mg/day 2-4 hr before exposure to UVA or sunlight. Therapy should be on alternate days & never on 2 consecutive days. Max dose: 0.6 mg/kg. Psoriasis Adult weighing >115 kg 70 mg, 91-115 kg 60 mg, 81-90 kg 50 mg, 66-80 kg 40 mg, 51-65 kg 30 mg, 30-50 kg 20 mg, <30 kg 10 mg. Doses should be administered 1.5-2 hr before exposure to UVA light, administered 2-3 times wkly (at least 48 hr apart).
Administration
Should be taken with food: Take w/ milk or after food as a single dose or in 2 divided doses (approx 30 min apart) to minimise GI side effects.
Contraindications
Skin diseases aggravated by sunlight, lupus erythematosus, porphyria, HTN, heart, liver or kidney failure; cataract; history of skin cancer, previous treatment w/ arsenic, ionizing radiation. Childn.
Special Precautions
Monitor stipulated gradual increase in duration of exposure to UV radiation. Avoid excessive exposure to sunlight after each treatment session. Use sunblock to avoid all risk of burning. Protect the eyes during treatment sessions & for 8-10 hr following treatment. Closely monitor for the potential long-term risk of photochemotherapy eg, skin ageing, changes in pigmentation, basal cell epithelioma & cataract risk. Women of childbearing age should use an effective form of contraception. Pregnancy & lactation.
Adverse Reactions
Severe edema, hypotension; nervousness, vertigo, depression; painful blistering, burning, peeling of skin, pruritus, freckling, hypopigmentation, rash, cheilitis, erythema, itching; nausea; loss of muscle coordination.
Drug Interactions
May produce additive photosensitizing effects w/ photosensitizing agents eg, anthralin, coal tar or its derivatives, griseofulvin, phenothiazine, nalidixic acid, halogenated salicylanilides (bacteriostatic soaps), sulfonamides, tetracyclines, thiazides, organic dyes eg, methylene blue, toluidine blue, rose bengal & methyl orange.
