Indications/Uses
Community-acquired pneumonia caused by Strep pneumoniae, including concurrent bacteremia, or Staph aureus (methicillin-susceptible strains). Nosocomial pneumonia caused by Staph aureus (methicillin-susceptile & resistant strains only), or Strep pneumoniae. Uncomplicated skin & skin structure infections caused by Staph aureus (methicillin-susceptible strains only) or Strep pyogenes. Complicated skin & skin structure infections (including diabetic foot infections) w/o concurrent osteomyelitis caused by Staph aureus (methicillin-susceptible & -resistant strains), Strep pyogenes or Strep agalactiae. Vancomycin-resistant Enterococcus faecium (VRE) infections including cases w/ concurrent bacteremia.
Dosage/Direction for Use
Complicated skin & structure infections, & community acquired pneumonia including concurrent bacteremia, hospital-acquired or healthcare-associated pneumonia Adult & adolescent ≥12 yr 600 mg every 12 hr for 10-14 days. Childn ≤11 yr 10 mg/kg every 8 hr for 10-14 days. Uncomplicated skin & skin structure infections Adult Recommended 600 mg or 400 mg every 12 hr for 10-14 days. Childn & adolescent ≥12 yr 600 mg every 12 hr for 10-14 days, 5-11 yr 10 mg/kg every 12 hr for 10-14 days, <5 yr 10 mg/kg every 8 hr for 10-14 days. VRE infections including concurrent bacteremia Adult & adolescent ≥12 yr 600 mg every 12 hr for 14-28 days. Childn ≤11 yr 10 mg/kg every 8 hr for 14-28 days. For all indications Neonate Initiate w/ 10 mg/kg every 12 hr. Consideration may be given to the use of 10 mg/kg every 8 hr.
Administration
May be taken with or without food.
Contraindications
Hypersensitivity. Concomitant use w/ MAOIs or use w/in 2 wk of MAOIs.
Special Precautions
Monitor CBCs wkly during treatment especially in patients receiving treatment for >2 wk, & in those who have preexisting myelosuppressions, are receiving concomitant drugs that produce bone marrow suppression, or have a chronic infection that was, or is being treated w/ concomitant anti-infective therapy; signs & symptoms of serotonin syndrome or neuroleptic malignant syndrome-like reactions; visual function in patients receiving prolonged treatment (>3 mth); glucose in diabetic patients. Discontinue treatment if myelosuppression worsens or develops. Concomitant use w/ serotonergic drugs. Do not use in patients w/ carcinoid syndrome or in patients receiving SSRIs, TCAs, serotonin 5-HT1 receptor agonists, meperidine, bupropion, or buspirone. Superinfection or C. difficile-associated diarrhea & colitis. Patients w/ uncontrolled HTN, pheochromocytoma, or thyrotoxicosis, or receiving direct or indirect-acting sympathomimetics, vasopressor agents or dopaminergic agents. Lactic acidosis. History of seizures. Pregnancy & lactation.
Adverse Reactions
Headache, dizziness, insomnia, vertigo; diarrhea, abdominal pain, constipation, dysgeusia, increased serum lipase, loose stool, nausea, oral candidiasis, pancreatitis, tongue discoloration, vomiting, decreased Hb, leukopenia, thrombocytopenia, anemia, eosinophilia, neutropenia; pruritus, skin rash; increased amylase, increased lactate dehydrogenase; vulvovag candidiasis; abnormal hepatic function tests, increased serum ALT, AST (adults), alkaline phosphatase, bilirubin & creatinine; fungal infection; BUN; fever.
Drug Interactions
Inhibits monoamine oxidase & interact w/ drugs that are metabolized by monoamine oxidase; alcohol (ethyl), apraclonidine, atomoxetine, atropine (ophth), BCG (intravesical), bezafibrate, buprenorphine, bupropion, buspirone, carbamazepine, cyclobenzaprine, cyproheptadine, dapoxetine, deferiprone, dexmethylphenidate, dextromethorphan, diethylpropion, dipyrone, epinephrine (oral inhalation), fentanyl, hydromorphone, isometheptene, levonordefrin, MAOIs, maprotiline, meperidine, mequitazine, methyldopa, methylene blue, methylphenidate, mianserin, mirtazapine, moclobemide, morphine (liposomal), morphine (systemic), nefazodone, nefopam, oxymorphone, pholcodine, pizotifen, reboxetine, SSRIs, serotonin 5-HT1D receptor agonists, serotonin/norepinephrine reuptake inhibitors, tapentadol, tetrabenazine, tetrahydrozoline (nasal), tianeptine, trazodone, TCAs, tryptophan. Food or beverages w/ high tyramine content.
