Indications/Uses
Myelodysplastic syndrome subtypes: Refractory anemia, or refractory anemia w/ ringed sideroblasts, excess blasts, excess blasts in transformation, chronic myelomonocytic leukemia. Adults ≥65 yr not eligible for haematopoietic stem cell transplantation w/ AML >30% marrow blasts.
Dosage/Direction for Use
IV/SC 1st treatment cycle: 75 mg/m2 daily for 7 days. Subsequently, repeat cycles every 4 wk, may be increased to 100 mg/m2 for min of 4-6 cycles.
Contraindications
Hypersensitivity to azacitidine or mannitol. Advanced malignant hepatic tumors.
Special Precautions
Perform CBC prior to each dosing cycle. Obtain liver chemistries & serum creatinine prior to initiation of therapy. Severe preexisting hepatic impairment. Renal impairment. Women of childbearing potential should avoid becoming pregnant during treatment. Men should not father a child during treatment. Pregnancy & lactation. Ped. Elderly (impaired renal function).
Adverse Reactions
Anemia &/or aggravated anemia, febrile neutropenia, leukopenia, neutropenia, thrombocytopenia; abdominal tenderness/pain, constipation, diarrhea, gingival bleeding, loose stools, mouth hemorrhage, nausea, stomatitis, vomiting, dyspepsia; chest/chest wall pain, inj site bruising, erythema, granuloma, pain, pigmentation changes, pruritus, reaction & swelling, lethargy, malaise, pyrexia, fatigue; nasopharyngitis, pneumonia, URTI, rhinitis, UTI; post-procedural hemorrhage; anorexia, hypokalemia; arthralgia, myalgia; dizziness, headache; anxiety, insomnia; dyspnea, exertional dyspnea, pharyngolaryngeal pain; dry skin, ecchymosis, erythema, rash, skin nodule, urticaria; hematoma, hypotension, petechiae; decreased wt; hematuria. Elevated serum creatinine, renal failure, renal tubular acidosis, hepatic coma; rigors.
