Amlodipine besylate
Indications/Uses
HTN; myocardial ischemia whether due to fixed obstruction (chronic stable angina), vasospasm/vasoconstriction angina (Prinzmetal's or variant angina) of coronary vasculature. Reduce risk of coronary revascularization & prevention of hospitalization due to angina w/ documented CAD limited to patients w/o heart failure or ejection fraction <40%.
Dosage/Direction for Use
HTN Elderly, patient w/ liver disease Initially 2.5 mg daily. Adult Initially 5 mg once daily. Maintenance: 5-10 mg once daily. Childn 6-17 yr 2.5-5 mg once daily. Stable & variant angina, CAD Adult 5-10 mg once daily. Angina Elderly, patient w/ liver disease 5 mg.
Administration
May be taken with or without food.
Contraindications
Special Precautions
Increased angina &/or MI; symptomatic hypotension w/ or w/o syncope. Peripheral edema w/in 2-3 wk of starting therapy; reflex tachycardia. Severe aortic stenosis &/or hypertrophic cardiomyopathy. Hepatic impairment. Pregnancy & lactation. Childn <6 yr.
Adverse Reactions
Peripheral edema. Flushing, palpitation; headache, dizziness, fatigue, somnolence; rash, pruritus; male sexual dysfunction; nausea, abdominal pain, dyspepsia, gingival hyperplasia; muscle cramps, weakness; dyspnea, pulmonary edema. Abnormal dreams, agitation alopecia, amnesia, anxiety, apathy, arrhythmia, ataxia, bradycardia, cardiac failure, cholestasis, depersonalization, depression, erythema multiforme, exfoliative dermatitis, extrapyramidal symptoms, gastritis, gynecomastia, hepatitis, hypotension, purpura, rash, skin discoloration, SJS, syncope, thrombocytopenia, tinnitus, increased transaminases, urticaria, wt loss, vertigo, xerophthalmia.
Drug Interactions
Increased level/effect of amifostine, antihypertensives, CYP1A2 substrate, hypotensive agent, Mg salt, nondepolarizing neuromuscular blocker, nitroprusside, phenytoin, rituximab, tacrolimus. Increased level/effect by α1-blocker, systemic antifungals (azole derivative), nondihydropyridine Ca channel blocker, systemic cyclosporine, moderate/strong CYP3A4 inhibitor, dasatinib, diazoxide, fluconazole, macrolides, Mg salt, MAOIs, pentoxifylline, phosphodiesterase-5 inhibitor, prostacyclin analogue, PIs, quinupristin, grapefruit juice, herbs (hypotensive properties). Decreased level/effect of clopidogrel, quinidine. Decreased level/effect by barbiturate, Ca salt, carbamazepine, strong CYP3A4 inducer, deferasirox, herbs (CYP3A4 inducers), methylphenidate, nafcillin, rifamycin derivative, yohimbine; St. John's wort. Concomitant use w/ dong quai (w/ estrogenic activity); ephedra, yohimbine, ginseng. Increased antihypertensive effect w/ garlic.
