Filgrastim (recombinant human granulocyte-colony stimulating factor, G-CSF)
Indications/Uses
Reduction in duration of neutropenia & incidence of febrile neutropenia in cytotoxic chemotherapy (except in chronic myeloid leukemia & myelodysplastic syndromes); reduction in duration of neutropenia (& associated sequelae) in myeloablative therapy followed by bone marrow transplantation; mobilization of peripheral blood progenitor cells for harvesting & subsequent analogous or allogeneic infusion; severe congenital, cyclic or idiopathic neutropenia & history of severe or recurrent infections; persistent neutropenia in advanced HIV infection.
Dosage/Direction for Use
Cytotoxic-induced neutropenia Adult & childn 500,000 u/kg SC/IV infusion over 30 min daily started not <24 hr after cytotoxic chemotherapy, continue until neutrophil count is normal usually up to 14 days. Myeloablative therapy followed by bone marrow transplantation 1 million u/kg IV infusion over 30 min or SC/IV infusion over 24 hr daily started not <24 hr after cytotoxic chemotherapy (& w/in 24 hr of bone marrow infusion), adjust according to neutrophil count. Mobilization of peripheral blood progenitor cells for autologous infusion Monotherapy: 1 million u/kg SC inj/infusion (24 hr) daily for 5-7 days. Adjunct to myelosuppresive chemotherapy: 500,000 u/kg SC inj daily until neutrophil count is normal. SC Mobilization of peripheral blood progenitor cells in normal donors for allogeneic infusion Adult <60 yr & adolescent >16 yr 1 million u/kg daily for 4-5 days. Severe chronic neutropenia Adult & childn Initially 1.2 million u/kg daily in sinlge or divided doses. Persistent neutropenia in HIV infection Initially 100,000 u/kg daily increased until neutrophil count is normal. Max: 400,000 u/kg daily.
Contraindications
Severe congenital neutropenia (Kostman's syndrome) w/ abnormal cytogenetics. Pregnancy & lactation.
Special Precautions
Pre-malignant or malignant myeloid conditions, monitor full blood counts & spleen size.
Adverse Reactions
GI disturbances, anorexia, headache, asthenia, fever, musculoskeletal pain, bone pain, rash, alopecia, inj site reactions & leucocytosis. Pulmonary infiltration.
