Carvedilol
Indications/Uses
Management of HTN & angina pectoris; adjunct to standard therapy in symptomatic heart failure.
Dosage/Direction for Use
Essential HTN Initially 12.5 mg daily for the 1st 2 days, continue w/ 25 mg once daily. May be increased to 25 mg bid for min of 14 days. Max: 25 mg as single dose or 50 mg total daily dose. Chronic stable angina Initially 12.5 mg bid for the 1st 2 days, continue w/ 25 mg bid. May be subsequently increased at interval of at least 2 wk up to max of 100 mg given in divided doses daily.
Administration
Should be taken with food.
Contraindications
Unstable/decompensated heart failure, liver dysfunction, 2nd- & 3rd-degree AV block (unless permanent pacemaker is in place), severe bradycardia (<50 bpm), sick sinus syndrome including SA block, severe hypotension (systolic BP <85 mmHg), cardiogenic shock; history of bronchospasm, asthma or other obstructive lung disorder.
Special Precautions
Worsening cardiac failure or fluid retention in chronic heart failure patients during up-titration. Concomitant use w/ digitalis glycosides. Severe hepatic impairment. Monitor liver function regularly.
Adverse Reactions
Hyperbilirubinemia, increased liver function tests.
Drug Interactions
Verapamil, diltiazem, class I antiarrhythmics; hypoglycemic therapy.
