Indications/Uses
Treatment of resp tract infections, ENT infections including otitis media, sinusitis, tonsillitis, pharyngitis, acute & chronic bronchitis, bronchiectasis w/ infection, bacterial pneumonia, lung abscess & post-op pulmonary infection; UTI including pyelonephritis, cystitis & asymptomatic bacteriuria; skin & skin structure infections including cellulitis, erysipelas, peritonitis & traumatic infection; septicemia; meningitis; gonorrhea (uncomplicated & complicated gonorrhea) especially for the patients in whom penicillin treatment is not recommended; bone & joint infections. Pre-op prophylaxis which reduce the incidence of certain perioperative & post-op infections in patients undergoing surgical procedures.
Dosage/Direction for Use
Adult 750 mg-1.5 g every 8 hr usually for 5-10 days. Life-threatening infections or infections due to less susceptible organisms 1.5 g every 6 hr. Bacterial meningitis Not exceeding 3 g every 8 hr. Uncomplicated gonococcal infection 1.5 g IM as a single dose at 2 different sites together w/ 1 g of oral probenecid. Preventive use for clean-contaminated or potentially contaminated surgical procedures 1.5 g IV prior surgery (approx ½-1.5 hr before the initial incision). Thereafter, 750 mg IV or IM every 8 hr when the procedure is prolonged. Preventive use during open chest surgery 1.5 g IV at the induction of anesth & every 12 hr thereafter for a total of 6 g. Ped patient >3 mth Most susceptible infections 50-100 mg/kg/day in equally divided doses every 6-8 hr. More severe or serious infections 100 mg/kg/day (not to exceed the max adult dosage). Bone & joint infections 150 mg/kg/day in equally divided doses every 8 hr. Bacterial meningitis 200-240 mg/kg/day in equally divided doses every 6-8 hr. Total daily dose for ped patient: Not exceeding 6 g.
Contraindications
Known allergy to the cephalosporins.
Special Precautions
Penicillin-allergic patients & who have had an anaphylactic shock to penicillin. Evaluate of renal status during therapy especially in seriously ill patients receiving the max doses. Renal insufficiency; receiving concurrent treatment w/ potent diuretics or aminoglycoside antibiotics. Discontinue in case of pseudomembranous colitis. Mild-to-moderate hearing loss in a few ped patients. +ve Coombs' test; false +ve reaction w/ Benedict's or Fehling's soln or w/ CLINITEST tab, false -ve result w/ ferricyanide test. Pregnancy & lactation. Ped patients <3 mth. Elderly ≥65 yr.
Adverse Reactions
Thrombophlebitis; diarrhea, nausea, pseudomembranous colitis; rash, pruritus & urticaria; decrease in Hb & hematocrit, transient eosinophilia, neutropenia & leukopenia; transient rise in ALT & AST, alkaline phosphatase, LDH & bilirubin levels; vomiting, abdominal pain, colitis, vaginitis including vag candidiasis, hepatic dysfunction including cholestasis, aplastic/hemolytic anemia, hemorrhage, triggering seizures, prolonged prothrombin time, pancytopenia & agranulocytosis.
Drug Interactions
Increased risk of nephrotoxicity w/ potent diuretics (eg, furosemide, ethacrynic acid & bumetanide), antineoplastics (eg, carmustine & streptozotocin) or aminoglycoside antibiotics. Incompatible w/ amikacin sulfate, gentamicin, kanamycin, tobramycin, neomycin, aureomycin HCl, tetracycline HCl, colistin Na methanesulfonate, polymyxin B sulfate, erythromycin gluconate, erythromycin lactobionate, lincomycin, soluble barbiturates, Ca chloride, Ca glucoheptonate, diphenhydramine HCl & other antihistamines, lidocaine, norepinephrine, metaraminol, ritalin, succinylcholine, penicillin, methicillin, hydrocortisone Na succinate, phenytoin Na, prochlorperazine, vit-B group, vit C & protein hydrolysate. False +ve reaction for glucose in the urine w/ Cu reduction tests (Benedict's or Fehling's soln or w/ CLINITEST tab), false -ve result w/ ferricyanide test.
