Oxaliplatin
Indications/Uses
Metastatic CRC after failure of treatment w/ fluoropyrimidines as monochemotherapy or along w/ fluoropyrimidines.
Dosage/Direction for Use
As single agent 130 mg/m2 as continuous IV infusion over 2-6 hr, may be repeated at an interval of 3 wk. In combination therapy Day 1: Administer 85 mg/m2 concurrently w/ leucovorin 200 mg/m2 by IV infusion over 2 hr, then fluorouracil 400 mg/m2 by IV inj over 2-4 min followed by fluorouracil 600 mg/m2 by IV infusion over 22 hr. Day 2: Administer leucovorin 200 mg/m2 by IV infusion over 2 hr, then fluorouracil 400 mg/m2 by IV inj over 2-4 min followed by fluorouracil 600 mg/m2 by IV infusion over 22 hr. Repeat regimen at interval of 2 wk.
Contraindications
History of severe allergy. Severe pre-existing peripheral neuropathy. Severe renal dysfunction (CrCl <30 mL/min). Pregnancy & lactation.
Special Precautions
Hypersensitivity to other platinum agents. Assess differential WBC count, Hb, platelet count, ALT, AST, bilirubin & creatinine levels before each Xalipat cycle. Probable carcinogen. Renal impairment.
Adverse Reactions
Sensory neuropathy; anaemia; fever; nausea & vomiting; liver function abnormalities; infections; alopecia; pharyngolaryngeal dysesthesia.
Drug Interactions
Potentiate irinotecan inhibition of acetylcholinesterase. Synergistic w/ fluorouracil & SN-38 (active irinotecan metabolite).
