Indications/Uses
Management of essential HTN. Angina pectoris. Symptomatic chronic heart failure.
Dosage/Direction for Use
Essential HTN Initially 12.5 mg once daily for 1st 2 days, then 25 mg once daily thereafter. May be increased subsequently at intervals of at least 2 wk up to max: 50 mg once or bd. Angina pectoris Initially 12.5 mg bd for 1st 2 days, then 25 mg bd. Thereafter, may be increased at intervals of at least 2 wk up to max: 100 mg bd. Symptomatic, stable, chronic heart failure Initially 3.125 mg bd for 2 wk, may be increased at intervals not <2 wk to 6.25 mg, 12.5 mg & 25 mg bd. Mild, moderate to severe chronic heart failure patient <85 kg Max: 25 mg bd, >85 kg Max: 50 mg bd.
Administration
Should be taken with food.
Contraindications
Hypersensitivity. Unstable/decompensated heart failure. 2nd & 3rd degree AV block, severe bradycardia (<50 bpm), sick sinus syndrome ie, SA block, severe hypotension (systolic BP <85 mmHg), cardiogenic shock, history of bronchospasm or asthma. Hepatic dysfunction.
Special Precautions
Hypersensitivity. Transient worsening of heart failure or fluid retention. Low BP (systolic BP <100 mmHg), ischemic heart disease, diffuse vascular disease, COPD, bronchospasm, DM, peripheral vascular disease, Raynaud's phenomenon, thyrotoxicosis, bradycardia, psoriasis, pheochromocytoma. Prinzmetal's variant angina. Patients undergoing general surgery. Contact lens wearers. Avoid abrupt w/drawal. Combination w/ digitalis glycosides. Concomitant use w/ verapamil- or diltiazem-type Ca-channel blockers, other antiarrhythmics, anesth. May affect ability to drive or operate machinery. Underlying renal insufficiency. Pregnancy. Not recommended during lactation. Childn <18 yr.
Adverse Reactions
Dizziness, headache, asthenia, fatigue. Bradycardia, postural hypotension, hypotension, edema; nausea, diarrhea, vomiting, abdominal pain; increased wt, hypercholesterolemia, hyper- & hypoglycemia, worsening of BP control; vision abnormalities, reduced lacrimation, eye irritation; asthma, dyspnea; pain in extremities.
Drug Interactions
Increased digoxin conc. Enhanced blood sugar-reducing effect of insulin or oral hypoglycemics. Reduced plasma conc by rifampicin. Increased AUC by cimetidine. Signs of hypotension &/or severe bradycardia w/ catecholamine-depleting agents eg, reserpine and MAOIs. Increased mean trough cyclosporine conc. Increased risk of AV conduction disturbances w/ verapamil- & diltiazem-type Ca-channel blockers or other antiarrhythmics. Potentiated BP- & heart rate-lowering effects w/ clonidine. Potentiated effect of other antihypertensives eg, α1-receptor antagonist. Synergistic -ve inotropic & hypotensive effects w/ anesth. Intensified BP-lowering effect w/ barbiturates, phenothiazines, TCAs, vasodilating drugs, alcohol.
