Indications/Uses
Abortion between 13th & 20th wk of gestation & in the following conditions related to 2nd trimester abortion: Failure of fetus expulsion by another method, premature membrane rupture in intrauterine methods w/ drug loss & insufficient or absent uterine activity, requirement of repeat intrauterine drug instillation for fetus expulsion, inadvertent or spontaneous membrane rupture in the presence of previable fetus & absence of adequate activity for expulsion. Postpartum hemorrhage due to uterine atony which has not responded to conventional methods of management.
Dosage/Direction for Use
Abortion Initially 250 mcg by deep IM inj then subsequently at 1½-3½ hr intervals depending on uterine response. Total dose: Not to exceed 12 mg; max duration: 2 days. Refractory postpartum uterine bleeding Initially 250 mcg by deep IM inj. Total dose: Not to exceed 2 mg (8 doses).
Contraindications
Hypersensitivity. Acute pelvic inflammatory disease. Active cardiac, pulmonary, renal or hepatic disease.
Special Precautions
Not to be used if fetus in utero has reached viability stage. Not to be considered as feticidal agent. History of asthma, hypotension or HTN, CV disease, anemia, jaundice, diabetes, epilepsy. Patients w/ scarred uteri; spontaneous/incomplete abortion; postpartum hemorrhage; chorioamnionitis. Not recommended in concomitant use w/ oxytocic agents. Renal or hepatic disease. Pregnancy & lactation. Ped patients. Fatal gasping syndrome in premature infants due to benzyl alcohol.
Adverse Reactions
Vomiting, diarrhea, nausea, flushing, chills, cough, headache, endometritis, hiccough, dysmenorrhea-like pain, paresthesia, backache; muscular, eye, inj site, epigastric & chest pain; breast tenderness, drowsiness, dystonia, asthma, tinnitus, vertigo, vasovagal syndrome, mouth dryness, hyperventilation, resp distress, hematemesis, taste alterations, UTI, septic shock, torticollis, lethargy, HTN, tachycardia, pulmonary edema, endometritis from IUCD, nervousness, nosebleed, sleep disorders, dyspnea, chest tightness, wheezing, posterior cervical perforation, weakness, diaphoresis, dizziness, blurred vision, excessive thirst, twitching eyelids, gagging, retching, dry throat, choking sensation, thyroid storm, syncope, palpitations, rash, upper resp infection, leg cramps, perforated uterus, anxiety, retained placental fragment, shortness of breath, fullness of throat, uterine sacculation, faintness, lightheadedness, uterine rupture.

Augmented activity of other oxytocic agents.
