Fluconazole
Indications/Uses
Cryptococcosis. Systemic, mucosal & vag candidiasis. Prevention of fungal infection in patients w/ malignancy. Dermatomycosis.
Dosage/Direction for Use
Cryptococcal meningitis & cryptococcal infections at other sites Adult 400 mg on 1st day followed by 200-400 mg once daily. Duration of treatment for cryptococcal meningitis: 6-8 wk. Childn 6-12 mg/kg daily. Prevention of relapse of cryptococcal meningitis in patient w/ AIDS, after receiving full course of primary therapy Daily dose: 200 mg. Candidemia, disseminated candidiasis & other invasive candidal infections 400 mg on 1st day followed by 200 mg daily, may be increased to 400 mg daily. Oropharyngeal candidiasis 50 mg once daily for 7-14 days or longer in patient w/ severely compromised immune function. Childn 3 mg/kg daily. Loading dose: 6 mg/kg may be used on 1st day. Atropic oral candidiasis associated w/ dentures 50-100 mg once daily for 7-14 days concurrently w/ local antiseptic measures to the denture. Other candidal infections of mucosa (except vag candidiasis) 50 mg daily for 14-30 days. Unusually difficult case of mucosal candidal infection May be increased up to 100 mg daily. Vag candidiasis 150 mg as single daily dose. Prevention of candidiasis 50-400 mg once daily. Dermal infections including tinea pedis, tinea corporis, tinea cruris & candida infections 150 mg once wkly or 50 mg once daily for 2-4 wk. Tinea pedis Up to 6 wk. Tinea versicolor 50 mg once daily for 2-4 wk. Infant 0-1 wk Same dose as older childn every 72 hr, 2-4 wk Same dose as older childn every 48 hr. Renal impairment Loading dose: 50-400 mg. CrCl >50 100% of recommended dose, CrCl 11-50 50% of recommended dose, patient on dialysis 1 dose after every dialysis.
Administration
May be taken with or without food.
Contraindications
Hypersensitivity to fluconazole or to related azole compd.
Special Precautions
Hypersensitivity. Patients w/ significant rise in liver enzymes. Pregnancy & lactation. Childn.
Adverse Reactions
GI disturbances, nausea, headache, abdominal pain, diarrhea, flatulence. Rash.
Drug Interactions
Increased prothrombin time w/ coumarin-type anticoagulants. Prolonged serum t½ of oral sulfonylureas. Increased plasma conc w/ hydrochlorothiazide. Increased phenytoin levels. Decreased t½ w/ rifampicin. Adverse CV effects w/ terfenadine. Decreased plasma clearance of theophylline. Increased plasma concentration of cyclosporin.
