Indications/Uses
Wound, pyogenic skin & soft tissue & resp tract infections. Penicillin-sensitive infections eg, generalised infections & septicaemia; acute & chronic osteomyelitis, subacute bacterial endocarditis & meningitis. Tetanus, actinomycosis, anthrax, rat-bite fever, listeriosis & severe Lyme disease. Gonococcal arthritis or endocarditis, congenital syphilis & neurosyphilis. Diphtheria, brain abscesses & pasteurellosis.
Dosage/Direction for Use
Adult 600-3,600 mg daily in 4-6 divided doses, depending on indication. Meningitis Up to 14.4 g IV daily in divided doses. Bacterial endocarditis 7.2-12 g IV daily in divided doses. Childn 1 mth-12 yr 100 mg/kg daily in 4 divided doses. Max: 4 g daily. Infant 1-4 wk 75 mg/kg daily in 3 divided doses. Newborn 50 mg/kg daily in 2 divided doses. Meningococcal disease Adult & childn >12 yr 2.4 g every 4 hr. Childn 1 mth-12 yr 180-300 mg/kg daily in 4-6 divided doses. Max: 12 g daily. Infant 1-4 wk 150 mg/kg daily in 3 divided doses. Newborn 100 mg/kg daily in 2 divided doses. Premature & neonate Not more frequent than every 8-12 hr. Renal insufficiency 0.6-1.2 g no more frequent than every 8-10 hr. Advanced liver disease associated w/ severe renal failure Reduce to 300 mg every 8 hr. Haemodialysis Additional 300 mg every 6 hr during procedure.
Contraindications
Hypersensitivity. History of penicillin allergy; cross allergy to other β-lactams eg, cephalosporins.
Special Precautions
Discontinue use if allergic reactions occur. Hypersensitivity to penicillins, cephalosphorins, carbapenems or other β-lactam agents. Overgrowth of nonsusceptible organisms w/ prolonged use. Pseudomembranous colitis. Diabetes. Hypokalemia, hypernatremia. Skin sensitization. Monitor electrolyte balance, blood count & renal function in high-dose prolonged treatment. Renal impairment. Pregnancy & lactation. Elderly.
Adverse Reactions
Jarisch-Herxheimer reaction. Rashes, fever, serum sickness.
Drug Interactions
Impaired efficacy of OCs. Reduced excretion of MTX. Inhibited tubular secretion & increased plasma conc by probenecid. Interfered w/ urinary glucose, Coombs', urinary or serum proteins & Guthrie tests.
