Alprazolam
Indications/Uses
Anxiety including anxiety neurosis, disorders, symptoms of anxiety; mixed anxiety (depression, including anxiety associated w/ depression); panic disorders including panic disorder w/ or w/o agoraphobia.
Dosage/Direction for Use
Individualized dosage. Anxiety Initially 0.75-1.5 mg daily in divided doses. Usual dose range: 0.5-4 mg daily in divided doses. Panic disorders 0.5-1 mg at bedtime or 0.5 mg 3 times daily. Dosage adjustment should be in increments ≤1 mg every 3-4 days. Geriatric 0.5-0.75 mg daily in divided doses, to be gradually increased if needed & tolerated.
Administration
Should be taken with food.
Contraindications
Hypersensitivity to benzodiazepines, alprazolam. Acute narrow angle glaucoma.
Special Precautions
Depression w/ psychosis features, in bipolar disorders or in endogenous depression. Habituation & emotional/physical dependence may occur. Patients who are prone to abuse drugs (eg, alcoholics & drug addicts). Not recommended for use in patients whose primary diagnosis is schizophrenia. Episodes of hypomania & mania in patients w/ depression. Dosage must be gradually tapered to preclude sequelae of rapid w/drawal. Avoid abrupt discontinuation. Administration to severely depressed or suicidal patients. Panic-related disorders. Advise patients not to operate motor vehicles or dangerous machinery while receiving Zolysan. Renal or hepatic impairment. Avoid use during pregnancy. Not to be taken during lactation. Childn <18 yr. Administer the smallest effective dose in elderly or debilitated patients to preclude the development of ataxia or over sedation.
Adverse Reactions
Depression, sedation, somnolence, ataxia, memory impairment, dysarthria, dizziness, headache, constipation, dry mouth, fatigue, irritability. Drowsiness & slurred speech in patients w/ panic-related disorders. Decreased appetite, confusional state, disorientation, decreased or increased libido, anxiety, insomnia, nervousness, balance disorder, abnormal coordination, attention disturbance, hypersomnia, lethargy, tremor, blurred vision, nausea, dermatitis, sexual dysfunction, decreased & increased wt.
Drug Interactions
Additive CNS depressant effects w/ other psychotropic medications, anticonvulsants, antihistaminics, ethanol, & other drugs producing CNS depression. Co-administration w/ ketoconazole, itraconazole, or other azole antifungals is not recommended. Caution & consider dose reduction when co-administered w/ nefazodone, fluvoxamine & cimetidine. Co-administration w/ fluoxetine, propoxyphene, OCs, diltiazem, or macrolide antibiotics eg, erythromycin & troleandomycin. Complex & time dependent interactions w/ HIV PIs (eg, ritonavir). Increased digoxin conc.
