Indications/Uses
Mild, moderate to severe infection caused by susceptible bacteria. Acute bacterial sinusitis & exacerbation from chronic bronchitis; nosocomial pneumonia; community-acquired pneumonia; chronic bacterial prostatitis; complicated skin infections, UTI & acute pyelonephritis.
Dosage/Direction for Use
250 or 500 mg every 24 hr. Community-acquired pneumonia 500 mg/24 hr for 7-14 days or 750 mg/24 hr for 5 days. Nosocomial pneumonia 750 mg/24 hr for 7-14 days. Acute bacterial sinusitis 500 mg/24 hr for 7-14 days or 750 mg/24 hr for 5 days. Acute exacerbation of chronic bronchitis 500 mg/24 hr for 7 days or 750 mg/24 hr for 3-5 days. Chronic bacterial prostatitis 500 mg/24 hr for 28 days. Complicated skin infections, complicated UTI, acute pyelonephritis 250-750 mg once daily for 7-14 days. Renal impairment CrCl >50 mL/min 250 mg or 500 mg/24 hr, CrCl 20-50 mL/min Initially 250 mg followed by 125 mg/24 hr or 500 mg followed by 250 mg/24 hr, CrCl 10-19 mL/min Initially 250 mg followed by 125 mg/48 hr or 500 mg followed by 125 mg/24 hr, CrCl <10 mL/min (w/ hemodialysis & CAPD) Initially 250 mg followed by 125 mg/48 hr or 500 mg followed by 125 mg/24 hr.
Administration
May be taken with or without food: Take at least 2 hr before or after Fe salts, antacids & sucralfate. Ensure adequate fluid intake.
Contraindications
Intolerance to class of fluoroquinolones, QTc prolongation. Patients w/ epilepsy & history of tendon impairment due to fluoroquinolone usage. Pregnancy & lactation. Childn & adolescent (<18 yr).
Special Precautions
Should be combined w/ other drugs to treat MRSA. Discontinue if tendinitis, tendon crack, enterocolitis pseudomembrane, hypersensitivity reactions & phototoxicity is suspected/occur. Certain drug therapy may predispose seizures/convulsion. Patients w/ impaired CNS, G6PD deficiency, psychiatric illness, peripheral neuropathy. May cause hyper or hypoglycemia in diabetic patients. Monitor coagulation test for co-administration w/ vit K antagonists eg, warfarin. May cause QT prolongation & electrolyte imbalance. Do not use w/ opiates. May impair the ability to drive or operate machine. Impaired renal (CrCl <80 mL/min) & liver function. Pregnancy & lactation. Elderly.
Adverse Reactions
Vomiting, bloating, headache, dizziness, insomnia, shivering. Diarrhea, nausea. Increased bilirubin.
Drug Interactions
Decreased absorption w/ Al-, Mg- or Fe-containing drugs. Reduced bioavailability w/ sucralfate. Reduction of brain seizure threshold w/ theophylline & NSAIDs; elimination w/ probenecid & cimetidine. Increased bleeding w/ vit K antagonist eg, warfarin.
