Ketorolac trometamol
Indications/Uses
Short-term management of moderate to severe acute post-op pain in max therapy of 5 days.
Dosage/Direction for Use
FC tab 10 mg every 4-6 hr as needed for pain. Max: 40 mg daily. Elderly Long interval dosage 6-8 hr is recommended. IM or IV bolus inj Adult Initially 10 mg followed by 10-30 mg 4-6 hrly. Max daily dose: Adult 90 mg. Elderly, impaired kidney function 60 mg. Administer via slow IV or IM bolus over a min of 15 sec.
Administration
Should be taken with food.
Contraindications
Hypersensitivity to ketorolac trometamol or other NSAIDs & those patients in whom acetosal or other prostaglandin synthesis inhibitors induce allergic reactions; aspirin (inj). History of active peptic ulceration or GI bleeding, asthma; suspected or confirmed cerebrovascular bleeding; haemorrhagic diatheses including coagulation disorders; complete or partial syndrome of nasal polyps, angioedema or bronchospasm; concurrent treatment w/ other NSAIDs, oxpentyfylline, probenecid or lithium salts; hypovolaemia from any cause or dehydration; neuraxial (epidural or intrathecal) administration; prophylactic administration pre-op or intraoperatively when haemostasis is critical. Patients undergone operations w/ high risk of haemorrhage or incomplete haemostasis; on anticoagulants including low-dose heparin (2,500-5,000 units 12 hrly); currently receiving acetosal. Moderate or severe renal impairment (serum creatinine >160 μmol/L). Pregnancy, labour, delivery or lactation. Childn <16 yr.
Special Precautions
History of GIT disease; cardiac decompensation & HTN. Not recommended for use as pre-op medication, for support of anesth or in obstet analgesia; chronic use. Potential for additive side effects w/ other NSAIDs. Closely monitor urine output, serum urea, creatinine & renal function. Inhibits platelet aggregation & may prolong bleeding time. Elderly >65 yr.
Adverse Reactions
GIT, CNS/musculoskeletal, urinary tract & kidney, CV/haematological disorders; asthma, dyspnea, pulmonary oedema; dermatological & hypersensitivity reactions; epistaxis, haematomata, post-op wound haemorrhage, increased bleeding time; abnormal LFTs, asthenia, oedema, inj site pain, wt gain, hepatitis, liver failure, fever.
Drug Interactions
Digoxin, warfarin, ibuprofen, naproxen, acetaminophen, phenytoin, tolbutamide, piroxicam, anticoagulant drug (eg, heparin). Reduced clearance & possible enhancement of methotrexate toxicity. Increased risk of renal disorder w/ ACE inhibitor & other NSAIDs, especially in dehydrated patients. Reduced diuretic response to furosemide in normovolaemic healthy patients. Sporadic cases of seizures w/ antiepileptic drugs (phenytoin, carbamazepine). Possible hallucination in patients taking psychoactive drugs. Concomitantly use w/ acetosal or other NSAIDs may worsen adverse reactions. Prolonged lithium & MTX kidney clearance. Reduced kidney elimination & increased ketorolac assay w/ probenecid.
