Paracetamol.
Description
Tab: Each PARACO tablet contains: Paracetamol 500 mg.
Syr: Each measurement spoonful (5 ml) of PARACO syrup contains: Paracetamol 120 mg.
Oral drops: Each ml contains: Paracetamol 100 mg.
Action
Pharmacology: Tab and syr: Analgesic-antipyretic.
As analgesic, works by increasing the pain threshold excitatory.
As antipyretic, thought to work directly on the heat regulating center in the hypothalamus.
Drops: The working mechanism of Paracetamol as an antipyretic agent is assumed to be based on the central effect equivalent to salicylic.
The antipyretic effect of Paracetamol can be described as follows: In the state of fever, the central heat regulator of the hypothalamus is estimated to undergo some trouble that the body temperature becomes elevated.
Paracetamol is believed to work out by making the central heat regulator back to normal again.
Paracetamol does not have a direct effect against blood vessels and cebacea gland, though this effect is central in characteristic, it does not perform a direct effect against the hypothalamus neurons.
Sweat formation comprises on outstanding effect after the administration of Paracetamol. However, it is not the result of its essential mechanism.
Paracetamol still reduces fever when the sweat formation is hindered by the administration of atropine.
The effect of fever reduction is supposed to take place by inhibiting the prostaglandin formation as its analgesic effect is similarly supposed to happen.
Prostaglandin E is a potent pyrogen which affect, when injected to the brain ventricle through hypothalamus anterior, cannot be inhibited by an antipyretic agent.
Pyrogen promotes the formation of Prostaglandin E, and the formation of this substance is still inhibited by Paracetamol.
Indications/Uses
Tab and syr: To relieve pain in a condition of headache, toothache and to get lower fever.
Drops: As an analgesic and antipyretic agent to relieve pains and reduce body temperature in the state of fever, mild and severe pains, as in the cases of tonsillectomy pains or reactions after immunization.
Dosage/Direction for Use
Tab: Adult: 1 - 2 tablets 3 - 4 times a day.
Children: 6 - 12 years old: ½ - 1 tablet 3 - 4 times a day.
Syr: 0 - 1 year old: ½ measurement spoonful (2.5 ml) 3 - 4 times a day.
1 - 2 years old: 1 measurement spoonful (5 ml) 3 - 4 times a day.
2 - 6 years old: 1 - 2 measurement spoonfuls (5 - 10 ml) 3 - 4 times a day.
6 - 9 years old: 2 - 3 measurement spoonfuls (10 - 15 ml) 3 - 4 times a day.
9 - 12 years old: 3 - 4 measurement spoonfuls (15 - 20 ml) 3 - 4 times a day.
Or on doctor's prescription.
Oral drops: 3 - 4 times a day for: Babies of 1 - 3 months old: 0.1 - 0.3 ml.
4 - 6 months old: 0.4 - 0.5 ml.
7 - 9 months old: 0.6 - 0.7 ml.
10 - 12 months old: 0.8 - 0.9 ml.
Children of 12 - 15 months old: 1.0 - 1.2 ml.

Contraindications
Patients with severe liver dysfunction.
Patients hypersensitive to this drug.
Special Precautions
Tab and syr: Careful use of this drug in patients with kidney disease.
After 2 days if fever does not decrease or after 5 days of pain did not disappear, immediately contact the Health Service Unit.
The use of this drug in patients who consume alcohol, can increase the risk of liver damage.
Oral drops: It should be given with caution when medicated to patients with renal impairment.
Side Effects
Tab and syr: Long-term use and large doses can cause liver damage.
Hypersensitivity reactions.
Oral drops: Hypersensitivity reactions (rarely found): skin rash, fever.
Blood dyscrasia (rarely found): haemolytic anaemia, neutropenia, leucopenia, thrombocytopenia, pancytopenia.
Hepatic disturbance: acute poisoning may cause a damage to the liver tissue (single dose of more than 15 g).
Drug Interactions
Oral drops: Alcohol, anticoagulant, chloramphenicol, aspirin, phenobarb, liver enzyme inducer, hepatotoxic agents.
Storage
Protect from direct light.
Tab and syr: Store below 30°C.
Oral drops: Store below 25°C.
