Indications/Uses
Herpes zoster. Accelerates resolution of zoster-associated pain, which includes acute & post-herpetic neuralgia. Herpes simplex infections of skin & mucous membranes including genital herpes. Herpes labialis (cold sores). Reduction of genital herpes transmission. Prophylaxis of cytomegalovirus (CMV) infection.
Dosage/Direction for Use
Adult Herpes zoster 1,000 mg tid for 7 days. Herpes simplex infections 500 mg bid. For recurrent episodes, treatment should be for 3 or 5 days. For initial episodes, treatment may have to be extended to 10 days. Herpes labialis (cold sores) 2 g bid for 1 day. Take 2nd dose 12 hr (no sooner than 6 hr) after the 1st dose. Prevention (suppression) of recurrences of herpes simplex infections Immunocompetent patient 500 mg once daily. Some patients w/ very frequent recurrences may gain additional benefit from the daily dose of 500 mg being taken as a divided dose (250 mg bid). Immunocompromised patient 500 mg bid. Reduction of genital herpes transmission Immunocompetent heterosexual 9 or fewer recurrences/yr: 500 mg once daily by the infected partner. Prophylaxis of CMV Adult & childn ≥12 yr 2 g qid. Initiate as early as possible post-transplant. Adjust dose according to CrCl. Duration: 90 days. May need to be extended in high risk patient.
Administration
May be taken with or without food: May be taken w/ meals to reduce GI discomfort.
Contraindications
Hypersensitivity to valaciclovir or acyclovir.
Special Precautions
Renal & hepatic impairment. Maintain adequate hydration. Elderly.
Adverse Reactions
Headache & nausea. Renal insufficiency, microangiopathic hemolytic anemia & thrombocytopenia in severely immunocompromised patients w/ high doses & prolonged periods but these have also been observed in patients not on valaciclovir having the same underlying conditions.
