Choline fenofibrate equiv to fenofibric acid
Indications/Uses
Adjunct to diet in combination w/ statin reduce triglycerides & increase in HDL-C in patients w/ mixed dyslipidemia & CHD or a CHD risk equivalent (other clinical forms of atherosclerotic disease: Peripheral arterial disease, abdominal aortic aneurysm & symptomatic carotid artery disease, diabetes, multiple risk factors that confer a 10-yr risk for CHD >20%) who are on optimal statin therapy to achieve their LDL-C goal. Reduce triglycerides in patients w/ severe hypertriglyceridemia. Reduce elevated LDL-C, total cholesterol, triglycerides & Apo B & increase HDL-C in patients w/ primary hyperlipidemia or mixed dyslipidemia.
Dosage/Direction for Use
135 mg once daily co-administered w/ 3-hydroxy-3-methyl-glutaryl-coenzyme A (HMG-CoA) reductase inhibitor.
Administration
May be taken with or without food: Swallow whole, do not open/chew/crush/dissolve.
Contraindications
Hypersensitivity to fenofibrate. Gallbladder disease; chronic or acute pancreatitis w/ the exception of acute pancreatitis due to severe hypertriglyceridemia; known phototoxic reaction during therapy w/ fibrates or ketoprofen. Severe renal insufficiency (CrCl <30 mL/min). Liver disease (including biliary cirrhosis & unexplained persistent liver function abnormality) & hepatic insufficiency.
Special Precautions
Myopathy in patient w/ diffuse myalgias, muscle tenderness or weakness, &/or marked elevations of creatine phosphokinase levels. Concomitant use w/ statins. Monitor liver & renal function regularly during therapy. Discontinue if enzyme levels persist >3x ULN. Pancreatitis. Renal & hepatic impairment. Elderly w/ renal impairment. Pregnancy & lactation. Childn <18 yr.
Adverse Reactions
Constipation, diarrhea, dyspepsia, nausea, fatigue, pain, nasopharyngitis, sinusitis, URTI, increased ALT, arthralgia, back pain, muscle spasms, myalgia, pain in extremity, dizziness, headache.
Drug Interactions
Potentiated effects of anticoagulants. Nephrotoxicity w/ cyclosporine. Increased risk of serious muscle toxicity w/ HMG CoA reductase inhibitors.
