Indications/Uses
Adjunctive treatment of severe systemic fungal infection caused by suscpetible strains of Candida or Cryptococcus. Chromoblastomycosis caused by susceptible fungi. In combination w/ amphotericin B for candidiasis & cryptococcosis due to rapid emergence of resistance to flucytosine in Candida & Cryptococcus.
Dosage/Direction for Use
50-150 mg/kg once daily in 4 equally divided doses at 6-hr intervals. Patients w/ renal impairment CrCl >40 mL/min 25 mg/kg/dose every 6 hr, 20-40 mL/min 25 mg/kg/dose every 12 hr, 10-20 mL/min 25 mg/kg/dose every 24 hr, <10 mL/min 25 mg/kg/dose every 48 hr. ESRD on intermittent hemodialysis 25-50 mg/kg every 48-72 hr after hemodialysis.
Administration
May be taken with or without food.
Contraindications
Special Precautions
Contraindicated in known hypersensitivity. May be harmful to kidneys, liver & bone marrow. Extreme caution in patients w/ bone marrow depression; hematologic disease, are treated or has a history of being treated w/ radiation or marrow suppressant drugs. Frequently monitor hepatic function & hematopoietic system during therapy; blood levels in patients w/ renal function impairment; hematologic status (WBC & platelet count) & liver function (alkaline phosphatase, ALT & AST) during treatment. Determine electrolytes, hematological & renal status prior to initiation of therapy. Pregnancy & lactation.
Adverse Reactions
Cardiac arrest, myocardial toxicity, ventricular dysfunction; ataxia, confusion, seizure, fatigue, hallucination, headache, hearing loss, paresthesia, parkinsonism, peripheral neuropathy, psychosis, pyrexia, sedation, vertigo, weakness; abdominal pain, anorexia, bilirubin elevation, diarrhea, dry mouth, duodenal ulcer, emesis, GI hemorrhage, hepatic dysfunction, hepatic enzymes elevation, acute hepatic injury, jaundice, nausea, ulcerative colitis; myelosuppression, agranulocytosis, aplastic anemia, anemia, eosinophilia, leukopenia, pancytopenia, thrombocytopenia; photosensitivity, pruritus, rash, TEN, urticaria; azotemia, creatinine elevation, BUN elevation, crystalluria, renal failure; chest pain, dyspnea, resp arrest; allergic reactions, hypoglycemia, hypokalemia.
Drug Interactions
May enhance adverse effect w/ amphotericin B. May diminish therapeutic effect w/ cytarabine. May increase risk of hematologic toxicity w/ zidovudine.
