Cefixime
Indications/Uses
Adult & ped patients w/ otitis media; pharyngitis & tonsillitis; sinusitis, acute & chronic bronchitis, pneumonia, acute infective exacerbation of COPD; uncomplicated & complicated UTI; uncomplicated gonorrhea & uncomplicated cervical, urethral, or rectal gonorrhea infections; typhoid.
Dosage/Direction for Use
Adult 100-400 mg daily as single dose or in 2 divided doses. Max: 400 mg daily. Otitis media 400 mg once daily or 200 mg every 12 hr for 10-14 days. Pharyngitis & tonsillitis 400 mg once daily or 200 mg every 12 hr for at least 10 days. Resp tract infections & UTI 400 mg once daily or 200 mg every 12 hr for 5-10 days. Uncomplicated gonorrhea 400 mg single dose. Uncomplicated cervical, urethral, rectal gonorrhea infection 400 mg single dose + azithromycin 1 g orally as single dose. Childn 6 mth to 12 yr 8 mg/kg daily in 1 or 2 divided doses. Childn 6 mth to 16 yr Typhoid 5-10 mg/kg bid for 14 days. Renal impairment w/ CrCl 21-59 mL/min or patient on hemodialysis 300 mg daily, ≤20 mL/min or patient on continuous ambulatory peritoneal dialysis 200 mg daily.
Administration
May be taken with or without food.
Contraindications
Hypersensitivity to cefixime or other cephalosporins.
Special Precautions
Hypersensitivity to penicillins. Stop treatment if fever, skin rash, blisters, detachment of skin & epithelial lining in oral cavity, throat, nose, genital organ & conjunctivitis occur. Patients w/ history of penicillin allergy; personal or familial history of bronchial asthma, rash, urticaria; poor oral nutrition & receiving parenteral nutrition; debilitated status. Vit K deficiency symptoms may develop. Superinfection w/ gm +ve bacteria in prolonged use. False +ve results in urinary glucose determinations; for ketones in urine nitroprusside tests. Renal impairment. Pregnancy & lactation. Elderly.
Adverse Reactions
Diarrhea, loose stools, abdominal pain, anorexia, nausea, vomiting, dyspepsia, flatulence, dry mouth; headache, dizziness, nervousness, insomnia, somnolence, malaise & fatigue; granulocytopenia, eosinophilia, thrombocytopenia; transient increase in AST (SGOT), ALT (SGPT), alkaline phosphatase, bilirubin & LDH; stridor, feeling unwell, dizziness, tinnitus, diaphoresis, candidiasis.
Drug Interactions
Increased prothrombin time w/ anticoagulant eg, warfarin. Increased plasma carbamazepine conc. Increased oral bioavailability w/ nifedipine. Increased effects w/ probenecid.
