Bromocriptine mesilate
Indications/Uses
Monotherapy or in combination w/ other antiparkinsonian drugs for all stages of idiopathic & post-encephalitic Parkinson's disease in adults. Conservative treatment of prolactinomas in adults & ped 7-17 yr. Adjunctive treatment/alternative to surgery or radiotherapy for acromegaly in adults & ped 7-17 yr. Supplementary to anti-estrogens eg, clomiphene for hyperprolactinaemia in men.
Dosage/Direction for Use
Adult Parkinsonism as dopamine agonist Initially 1.25 mg (½ tab) daily preferably in the evening for 1st wk. Prolactinomas 1.25 mg (½ tab) 2 or 3 times daily, gradually increased to several tab as required. Acromegaly Initially 1.25 mg (½ tab) 2 or 3 times daily, gradually increased to 10-20 mg daily depending on clinical response. Hyperprolactinaemia in men 1.25 mg (½ tab) 2 or 3 times daily, gradually increased to 5-10 mg daily. Ped 7-17 yr Prolactinomas Ped >7 yr 1.25 mg (½ tab) 2 or 3 times daily, gradually increased to several tab as required. Max daily recommended dose: Adolescent 13-17 yr 20 mg, childn 7-12 yr 5 mg. Acromegaly Ped >7 yr Initially 1.25 mg (½ tab) 2 or 3 times daily, gradually increased to several tab daily depending on clinical response. Max daily recommended dose: Adolescent 13-17 yr 20 mg, childn 7-12 yr 10 mg.
Administration
Should be taken with food.
Special Precautions
Greater sensitivity; decreased cardiac, hepatic or renal function in elderly ≥65 yr.
Adverse Reactions
Nasal congestion, somnolence. Headache, dizziness, nausea, constipation, vomiting.
Drug Interactions
Concomitant use w/ strong CYP3A4 substrates/inhibitors eg, azole antimycotics, HIV PIs. Increased plasma levels w/ macrolide antibiotics eg, erythromycin or josamycin; octreotide. HTN & severe headache w/ phenylpropanolamine. Additive pharmacological effects w/ sumatriptan. Increased dopamine stimulant activity w/ ergot alkaloids. Reduce activity w/ dopamine receptor antagonists eg, antipsychotics, phenothiazines, butyrophenones, thioxanthenes; metoclopramide & domperidone. Reduced tolerance by alcohol.
