Adult Start w/ 20 mg once daily, may be increased to a max 40 mg/day or hydrochlorothiazide therapy may be added, if necessary. Effect may be seen w/in 2 wk & max by 8 wk after initiating therapy.
Intravascular vol depletion, conditions w/ stimulation of the renin-angiotensin-aldosterone system, renovascular HTN, renal impairment & kidney transplantation, hyperkalemia, aortic or mitral valve stenosis; obstructive hypertrophic cardiomyopathy, primary aldosteronism, hepatic impairment, lithium. Excessive BP decrease in patients w/ ischaemic heart disease or ischaemic cerebrovascular disease could result in MI or stroke.
