Norepinephrine bitartrate
Indications/Uses
BP control in certain acute hypotensive states (eg, pheochromocytomectomy, sympathectomy, poliomyelitis, spinal anesth, MI, septicemia, blood transfusion, & drug reactions). Adjunctive treatment of cardiac arrest & profound hypotension.
Dosage/Direction for Use
IV infusion Given into a large vein. Initially 8-12 mcg/min. Adjust rate of flow to establish & maintain a low normal BP 80-100 mmHg systolic. Maintenance dose: 2-4 mcg/min.
Contraindications
Patients who are hypotensive from blood vol deficits except during emergencies; w/ mesenteric or peripheral vascular thrombosis; profound hypoxia or hypercarbia. Concomitant use w/ cyclopropane & halothane anesth.
Special Precautions
May cause allergic-type reactions including anaphylactic symptoms & life-threatening or less severe asthmatic episodes. Sulfite sensitivity more frequently in asthmatic than in non-asthmatic patient. Not to be used in patients w/ hypotension, diabetes, cardiac or thyroid disease. May occur occlusive vascular diseases (eg, atherosclerosis, arteriosclerosis, diabetic endarteritis, Buerger's disease) in lower than in upper extremity; gangrene in lower extremity when infusions were given in an ankle vein. May result to severe, prolonged HTN in patients receiving MAOIs or antidepressants of triptyline or imipramine types. Possible high BP w/ pressor agent overdose. Frequently check infusion site for free flow. Consider changing infusion site at intervals to allow effects of local vasoconstriction to subside if blanching occurs. Gradually reduce & avoid abrupt w/drawal of treatment. Avoid catheter tie-in technique if possible; contact w/ Fe salts, alkalis, or oxidizing agents. Pregnancy & lactation. Ped patients. Elderly ≥65 yr. Not to be administered into the leg veins of elderly patients.
Adverse Reactions
Ischemic injury; bradycardia, arrhythmias; anxiety, transient headache; resp difficulty; inj site extravasation necrosis.
Drug Interactions
Increased cardiac autonomic irritability w/ cyclopropane & halothane anesth. Concomitant use w/ MAOIs or antidepressants of triptyline or imipramine types.
