Norepinephrine bitartrate
Indications/Uses
BP control in certain acute hypotensive states (eg, pheochromocytomectomy, sympathectomy, poliomyelitis, spinal anesth, MI, septicemia, blood transfusion, & drug reactions). Adjunctive treatment of cardiac arrest & profound hypotension.
Dosage/Direction for Use
Administer as a single dose of 8 mg in 1,000 mL of dextrose 5% or normal saline 0.9% inj in a ratio of 2-3 mL/min as IV drip. Maintenance dose: 0.5-1 mL/min.
Contraindications
Allergenic cross-sensitivity w/ vasopressors. Hypotension from hypovolemia. Mesenteric or peripheral vascular thrombosis. Concomitant use w/ cyclopropane or halothane anesth.
Special Precautions
Patients receiving MAOIs or triptyline/imipramine type antidepressants. Assure adequate circulatory vol. Avoid HTN; extravasation; infusion into leg veins in elderly patients & those suffering from occlusive disorders (eg, atherosclerosis, arteriosclerosis, diabetic endarteritis, Buerger disease); catheter tie-in technique. Closely monitor BP, IV site & adjust infusion rate. Patients w/ profound hypoxia or hypercarbia. Ensure proper needle or catheter placement prior to, & during infusion. Administer phentolamine when extravasation occurs. Patients w/ asthma or sulfite allergy. Gangrene in a lower extremity. Pregnancy & lactation. Elderly ≥65 yr.
Adverse Reactions
Bradycardia, cardiac arrhythmia, stress cardiomyopathy, peripheral vascular insufficiency; anxiety, transient headache; dyspnea. Peripheral gangrene & ischemia.
Drug Interactions
Increased cardiac autonomic irritability w/ cyclopropane & halothane anesth. May cause severe prolonged HTN w/ MAOIs or triptyline/imipramine type antidepressants. Increased levels w/ SSRIs/SNRIs. Decreased levels w/ spironolactone.
