Indications/Uses
Serious or life-threatening susceptible bacterial infections eg, lower resp tract; bacteremia, septicemia (including neonatal sepsis), complicated UTIs, skin & skin structure & intra-abdominal (including peritonitis & intraabdominal abscess). Initial therapy w/ penicillin- or cephalosporin-type drug in serious infections of unknown cause. In combination w/ carbenicillin or ticarcillin for life-threatening infections caused by Pseudomonas aeruginosa. In conjunction w/ penicillin-type drug for endocarditis caused by Strep sp.
Dosage/Direction for Use
IM/IV Adult Complicated UTI & serious systemic infections 4-6 mg/kg daily (1.3-2 mg/kg every 8 hr or 2-3 mg/kg every 12 hr). Life-threatening infections Up to 7.5 mg/kg daily administered in 3 equal doses every 8 hr. Reduce to ≤4-6 mg/kg daily as clinically indicated, usually w/in 48 hr. Childn 6-7.5 mg/kg daily (2-2.5 mg/kg every 8 hr). Infant & neonate >1 wk 7.5-9 mg/kg daily (2.5-3 mg/kg every 8 hr). Premature or full-term neonate <1 wk 6 mg/kg daily (3 mg/kg every 12 hr). Treatment duration: 7-14 days. Renal impairment Adjusted total daily dose: 1 dose at 24-hr intervals, or as 2 or 3 equally divided doses at 12- or 8-hr intervals, respectively. Max: 3.25 mg/kg each individual dose. Adults undergoing hemodialysis 2 mg/kg at the end of each dialysis period. Childn undergoing hemodialysis 2-2.5 mg/kg, depending on severity of infection.
Contraindications
History of hypersensitivity to netilmicin or aminoglycosides. Pregnancy & lactation.
Special Precautions
Discontinue if renal impairment or ototoxicity occurs. Dehydration & advanced age. Patients w/ neuromuscular disorders (eg, myasthenia gravis, parkinsonism, infant botulism). Monitor renal & 8th cranial nerve function at onset of therapy for patients w/ known or suspected renal impairment & those who develop signs of renal dysfunction; drug serum conc. Avoid concomitant use w/ other ototoxic, neurotoxic or nephrotoxic drugs.
Adverse Reactions
Renal toxicity, neurotoxicity manifested as auditory & vestibular ototoxicity, headache, fever, visual disturbances, blurred vision, apnea, disorientation, neuromuscular blockade, vomiting, diarrhea, rash, hypotension.
