Norepinephrine bitartrate
Indications/Uses
BP control in acute hypotension.
Dosage/Direction for Use
IV infusion Adult Initially 2-3 mL (8-12 mcg)/min. Maintenance: 0.5-1 mL (2-4 mcg)/min. Max dose: Elderly, adult, adolescent 30 mcg/min IV continuous infusion.
Contraindications
Patients who are hypotensive; w/ peripheral or mesenteric thrombosis; profound hypoxia or hypercarbia. Concomitant use w/ cyclopropane & halothane anesth.
Special Precautions
Sulfite sensitivity in asthmatic patients. Occlusive vascular disease; preexisting hyperthyroidism or HTN. Risk of ventricular tachycardia or fibrillation during cyclopropane & halothane anesth. Monitor BP every 2 min from time of administration until desired BP is obtained, then every 5 min thereafter to avoid HTN. Avoid catheter tie-in technique; extravasation into tissues. Concomitant use w/ MAOIs, triptyline- or imipramine-type antidepressants. Hepatic & renal impairment. Pregnancy & lactation. Not recommended in childn. Not to be used into leg veins of elderly.
Adverse Reactions
Cardiac arrest & arrhythmia. HTN associated w/ bradycardia, headache, peripheral ischemia; severe tissue damage & necrosis; nausea, vomiting; confusion, tremor; anxiety, restlessness; urinary retention; dyspnea, resp difficulty; transient headache; ischemic injury; plasma vol depletion (prolonged use).
Drug Interactions
Increased risk of severe HTN w/ non-cardioselective β-blockers. Enhanced vasopressor effect of direct acting α-/β-agonists. Enhanced effects by entacapone catechol-o-methyl transferase (COMT) inhibitors. Enhanced vasopressor effect w/ MAOIs; TCAs. Concomitant use w/ cyclopropane & halothane anesth.
