Indications/Uses
Prophylaxis & treatment of venous thrombosis, pulmonary embolism & thrombolic disorders. Prophylaxis of thromboembolic complication associated w/ atrial fibrillation. Adjunct in prophylaxis of systemic embolism after MI.
Dosage/Direction for Use
Initially 2-5 mg daily. Maintenance: 2-10 mg daily. Duration of therapy: Individualized.
Administration
May be taken with or without food.
Contraindications
Hypersensitivity. Hemorrhagic tendencies or blood dyscrasias; recent or contemplated CNS or eye surgery/traumatic surgery resulting in large, open surfaces; GI & resp tract or GUT bleeding; aneurysm; pericarditis & pericarditis effusion; bacterial endocarditis; spinal puncture; lumbar block anesth. Women of childbearing potential. Pregnancy.
Special Precautions
Risk of hemorrhage; necrosis & gangrene of skin & other tissues. Protein C & S deficiencies. Concomitant administration w/ NSAIDs eg, aspirin. Birth malformations in childn born to mothers receiving treatment during pregnancy. Childn <18 yr. Elderly (lower initiation & maintenance dose recommended).
Adverse Reactions
Hypersensitivity/allergic reactions. Fatal or non fatal hemorrhage from any organ/tissue, paralysis, paresthesia, headache, chest, abdomen, joint, muscle or other pain, dizziness, shortness of breath, difficult breathing/swallowing, unexplained swelling & shock, weakness, hypotension; necrosis of skin & other tissues.
Drug Interactions
Increased effect w/ acetaminophen, androgens, capecitabine, cephalosporins, disulfiram, fibric acids, glucagon, NSAIDs, COX-2 selective, propoxyphene, quinolones, SSRIs, sulfonamides, thyroid hormones, zafirlukast, zileuton, allopurinol, amiodarone, antifungals, cimetidine, HMG-CoA reductase inhibitors, metronidazole, omeprazole, propafenone, quinidine, macrolides, tetracyclines, vit E. Decreased effect w/ OC, estrogens, griseofulvin, aminoglutethimide, barbiturates, carbamazepine, glutethimide.
