Manidipine HCl
Indications/Uses
Mild to moderate essential HTN & HTN w/ renal impairment.
Dosage/Direction for Use
Adult 10-20 mg once daily. Starting dose: 10 mg daily. May be increased to 20 mg daily if antihypertensive effect is inadequate after 2-4 wk. Elderly 10 mg daily. Patients w/ mild hepatic dysfunction Max: 10 mg daily.
Administration
Should be taken with food: Take after breakfast.
Contraindications
Hypersensitivity to manidipine & any dihydropyridine agents. MI in the 1st 4 wk; unstable angina pectoris, untreated heart failure, galactose intolerance, glucose-galactose malabsorption, Lapp lactase deficiency. Concomitant use w/ CYP3A4 inhibitors & inducers. Moderate to severe hepatic dysfunction. Severe renal dysfunction (CrCl <10 mL/min). Pregnancy. Childn.
Special Precautions
Possible increased coronary risk in coronary stable patients. Patients w/ isolated-right-sided heart failure, left ventricular dysfunction, left ventricle obstruction of the outflow channel, sick sinus syndrome. Mild hepatic impairment. Lactation. Elderly.
Adverse Reactions
Facial hot flushes, feeling warmth, palpitation, peripheral edema, tachycardia, hypotension, HTN, chest pain; pruritus, rash, eczema, erythema; nausea, vomiting, anorexia, stomach discomfort, heartburn, enlarged feeling of abdomen, diarrhea, constipation, xerostomia, gastralgia, abdominal pain; leukopenia, prolonged bleeding time; headache, dizziness, vertigo, numbness, paresthesia, weakness, Parkinsonian symptoms; irritability, somnolence; elevation of GOT, GPT, γ-GPT, LDH, alkaline phosphatase, BUN, serum creatinine.
Drug Interactions
May intensify the action of other antihypertensive drugs. Action may be intensified w/ cimetidine. May have additive effect on AV node conduction, & increased risk of bradycardia & advanced or complete heart block w/ digoxin. Concomitant use w/ amiodarone. Increased conc & toxicity w/ dalfopristin/quinupristin.
