Indications/Uses
Dosage/Direction for Use
Adult Initially 15 mg or 30 mg daily. Maintenance dose: 15-45 mg daily.
Administration
May be taken with or without food: Swallow whole w/ sufficient amount of fluid, do not chew.
Contraindications
Special Precautions
Discontinue if fever, throat pain, stomatitis, other signs & symptoms suggestive of infection occur; in the presence of jaundice; in patient entering manic phase. History of suicide-related events, suicide ideation; mania/hypomania; seizures. Bone marrow depression usually after 4-6 wk of treatment. Exacerbation of psychotic symptoms; intensified paranoid thoughts; switch to manic phase; akathisia/psychomotor restlessness. Avoid abrupt treatment cessation. Patients w/ epilepsy or organic brain syndrome; heart disease (eg, conduction disturbances, angina pectoris or recent MI); hypotension; micturition disturbances (eg, prostatic hyperplasia); acute narrow angle glaucoma & elevated IOP; DM. Galactose intolerance, Lapp lactase deficiency or glucose-galactose malabsorption. Concomitant use w/ medications known to cause hyponatraemia; serotonergic active substances. May affect ability to drive & use machines. Hepatic & renal impairment. Not to be used during pregnancy. Not recommended during lactation. Increased risk of persistent pulmonary HTN in newborn. Not to be used in childn & adolescents <18 yr. Elderly.
Adverse Reactions
Increased wt & appetite, hypertriglyceridaemia; somnolence, sedation, headache; dry mouth. Abnormal dreams, confusion, anxiety, insomnia; lethargy, dizziness, tremor; orthostatic hypotension; nausea, diarrhoea, vomiting; exanthema; arthralgia, myalgia, back pain; peripheral oedema, fatigue. SJS, bullous dermatitis, erythema multiforme, TEN. Childn: Hypertriglyceridaemia; urticaria.
Drug Interactions
Concomitant use w/ MAOIs, CYP3A4 inhibitors, HIV PIs, azole antifungals, erythromycin & nefazodone. Serotonin-associated effects w/ serotonergic active substances eg, L-tryptophan, triptans, tramadol, linezolid, SSRIs, venlafaxine, lithium & St. John's wort. Increased sedating properties of benzodiazepines & other sedatives. Increased CNS depressant effect of alcohol. Increased INR w/ warfarin. Increased clearance w/ carbamazepine & phenytoin. Increased peak plasma levels & AUC w/ ketoconazole. Increased mean plasma conc w/ cimetidine.
