Ca polystyrene sulfonate
Dosage/Direction for Use
Adult Oral 15 g tid to qid. Rectal 30 g daily given as susp in 100 mL of 2% methylcellulose '450' & water 100 mL retained for at least 9 hr. Oral Childn Acute hyperkalemia 1 g/kg daily in divided doses. Reduce to maintenance dose of 500 mg/kg daily in divided doses. Rectal Childn & neonates Similar w/ oral dose suggested for childn.
Administration
Should be taken on an empty stomach.
Contraindications
Hypersensitivity. Hypercalcemia & ventricular fibrillation; hyperparathyroidism, multiple myeloma, sarcoidosis or metastatic carcinoma who may present w/ renal failure w/ hypercalcemia.
Special Precautions
Not recommended to be given orally in neonates. Discontinue use in clinically significant constipation. Electrolyte disturbances including hypokalemia. Sudden increase of Ca in renal impairment &/or dialysis patients. Closely monitor electrolytes during therapy & discontinue if K ≤5 mmol/L (5 mEq/L). Monitor Ca wkly or more frequently. Avoid Mg-containing laxatives; concomitant use of rectal sorbitol. Pregnancy & lactation.
Adverse Reactions
Constipation & headache. Mild hypercalcemia (Ca >10.5 mg/dL) manifests as anorexia, nausea & vomiting. More severe hypercalcemia (Ca >12 mg/dL) associated w/ confusion, delirium, stupor & coma.
Drug Interactions
May reduce absorption of tetracycline (oral), bisphosphonates (eg, alendronate, etidronate, ibandronate, risedronate) & oral Fe prep. Synergistic effects & arrhythmia w/ cardiac glycosides. Reduced oral bioavailability of fluoroquinolones eg, ciprofloxacin.
