Indications/Uses
Acute & maintenance treatment of major depressive disorder (MDD) in adults & adolescents 12-17 yr. Acute treatment of generalized anxiety disorder (GAD) in adults. Panic disorder w/ or w/o agoraphobia. Social anxiety disorder. Obsessive-compulsive disorder.
Dosage/Direction for Use
Adult MDD, GAD 10 mg once daily, may be increased to max of 20 mg daily after a min of 1 wk. Panic disorder w/ or w/o agoraphobia Initially 5 mg for the 1st wk, thereafter increased to 10 mg daily. May be further increased up to max of 20 mg daily, dependent on individual patient response. Social anxiety disorder 10 mg once daily, depending on individual patient response may be increased to max: 20 mg daily. Obsessive-compulsive disorder Initially 10 mg once daily, depending on individual patient response, may be increased to max: 20 mg daily. Elderly (>65 yr) Initially 5 mg once daily, depending on individual patient response, may be increased to 10 mg daily. Childn & adolescents (12-17 yr) MDD Initially 10 mg once daily, may be increased to 20 mg daily after a min of 3 wk. Mild or moderate hepatic impairment, poor CYP2C19 metabolisers Initially 5 mg daily for the 1st 2 wk, may be increased to 10 mg daily.
Administration
May be taken with or without food.
Contraindications
Hypersensitivity to escitalopram & citalopram. Concurrent or recent use (ie, w/in 2 wk) w/ MAOIs. Concomitant use w/ pimozide.
Special Precautions
Discontinue use if signs & symptoms of serotonin syndrome occur. Increased risk of bleeding events; risks associated w/ electroconvulsive therapy. Previous seizure disorder or condition predisposing to seizures eg, brain damage, alcoholism, or concurrent use w/ drugs that lower seizure threshold. Hyponatremia/syndrome of inappropriate antidiuretic hormone secretion, vol depletion; mild papillary dilation leading to narrow-angle glaucoma; females w/ preexisting QT prolongation or other risk factors eg, hypokalemia, other cardiac disease. Patients w/ history of falls or fractures; mania. Monitor for clinical worsening, suicidality & unusual changes in behavior; Na levels during initiation of therapy & dose adjustment periods. Evaluate patients who have not had an iridectomy for narrow-angle glaucoma risk factors. Avoid abrupt discontinuation. Concomitant use w/ aspirin, NSAIDs, warfarin, other anticoagulants; CNS depressants. Combination w/ other serotonergic (eg, triptans, TCAs, fentanyl, lithium, tramadol, buspirone, St. John's wort, tryptophan) or MAOIs (eg, linezolid & IV methylene blue). Severe renal (CrCl <20 mL/min) or hepatic impairment. Pregnancy & lactation. Increased risk of suicidal thinking & behavior in childn, adolescents & young adults (18-24 yr) w/ MDD & other psychiatric disorders. Elderly.
Adverse Reactions
Abnormal dreams, anorgasmia, dizziness, drowsiness, fatigue, headache, insomnia, lethargy, paresthesia, yawning; diaphoresis; decreased libido, menstrual disease; abdominal pain, constipation, decreased appetite, diarrhea, dyspepsia, flatulence, nausea, toothache, vomiting, xerostomia; ejaculatory disorder, impotence; neck & shoulder pain; flu-like symptoms, rhinitis, sinusitis. Childn: UTI; back pain; nasal congestion.
Drug Interactions
Increased peak plasma conc & AUC of CYP2D6 isoenzyme substrates (eg, desipramine, metoprolol) & cimetidine. Increased bleeding w/ warfarin or other anticoagulants. Serotonin syndrome or NMS-like reactions w/ antipsychotics or other dopamine antagonists, 5-HT1 receptor agonists (eg, almotriptan, eletriptan, frovatriptan, naratriptan, rizatriptan, sumatriptan, zolmitriptan), MAOIs (eg, INH, linezolid), citalopram, sibutramine. Concomitant use w/ alcohol, CNS-active drug, lithium.
