Indications/Uses
Monotherapy in partial seizures w/ & w/o secondary generalization in adults & childn ≥12 yr. Adjunctive therapy in partial seizures w/ & w/o secondary generalization in adults & childn ≥3 yr. Neuropathic pain in patient ≥18 yr.
Dosage/Direction for Use
Epilepsy Max interval between tid dose: 12 hr. Adult & childn >12 yr Maintenance dose: 900-3,600 mg daily. Initially 300 mg tid on day 1 or w/ 3 days titration of 300 mg once daily on day 1, 300 mg bid on day 2, 300 mg tid on day 3. Max: 3,600 mg daily in 3 divided doses. Childn 5-12 yr Maintenance dose: 25-35 mg/kg daily, 3-4 yr 40 mg/kg daily, 3-12 yr Initially 10-15 mg/kg daily. All doses to be given in 3 divided doses. Neuropathic pain Adult Initially 900 mg daily in 3 divided doses or w/ 3 days titration of 300 mg once daily on day 1, 300 mg bid on day 2, 300 mg tid on day 3. Max: 3,600 mg daily as tid. Renal impairment w/ CrCl ≥80 mL/min 900-3,600 mg daily as tid, 50-79 mL/min 600-1,800 mg daily as tid, 30-49 mL/min 300-900 mg daily as tid, 15-29 mL/min 150 mg (to be administered as 300 mg every other day) to 600 mg daily as tid, <15 mL/min 150 mg (to be administered as 300 mg every other day) to 300 mg daily as tid. Patient undergoing hemodialysis Loading dose: 300-400 mg, then 200-300 mg following each 4 hr of hemodialysis.
Administration
May be taken with or without food.
Contraindications
Special Precautions
Discontinue treatment if signs & symptoms of severe hypersensitivity reactions eg, DRESS & anaphylaxis occur. Avoid abrupt w/drawal. Not for absence seizures. May cause hematologic disorder; CNS depression. Not to be taken w/ alcohol or alcoholic products. Potentiated effects w/ other sedatives or ethanol. May affect ability to drive & use machines. Liver disease. Severe renal dysfunction. Not to be used in pregnancy. Lactation. Increased incidence of CNS-related adverse effects, including emotional ability, hostility, thought disorder, & hyperkinesia in ped patients 3-12 yr. Childn <3 yr.
Adverse Reactions
Fatigue, infection, viral infection, headache, accidental injury, abdominal & back pain, increased wt, peripheral edema, fever; diarrhea, dry mouth or throat, constipation, nausea, vomiting, flatulence, dyspepsia, dental abnormalities, increased appetite; wt gain, hyperglycemia; dizziness, somnolence, ataxia, abnormal thinking, gait & coordination, incoordination, amnesia, hypoesthesia, nystagmus, tremor, nervousness, dysarthria, depression, twitching, hostility, emotional ability, hyperkinesia; pharyngitis, rhinitis, coughing, bronchitis, resp tract infection; rash, abrasion, pruritus; amblyopia, conjunctivitis, diplopia, otitis media; vasodilation; leucopenia; myalgia, fracture; impotence; decreased WBC.
Drug Interactions
Reduced bioavailability w/ antacids. Increased mean AUC w/ morphine. Increased mean absorption w/ naproxen. May increase CNS depression w/ ethanol. False +ve w/ Ames N-Multistix SG dipstick test.
